摘  要:目的探讨胎儿宫内窘迫后脐血甲状腺激素水平的变化及意义。方法新生儿出生后取脐静脉血,采用放射免疫法进行甲状腺激素(T4、TSH)测定,并按是否有宫内窘迫史及出生前宫内窘迫是否得到纠正分组比较。结果有宫内窘迫史但出生前已得到及时纠正,生后未发生新生儿窒息(Apgar评分1min〉7分),脐血T4、TSH与对照组比较差异无统计学意义;有宫内窘迫史,胎儿缺氧较久,出生前未得到纠正致新生儿窒息组,脐血T4水平随窒息程度的加重而下降,TSH水平随窒息程度的加重而升高。结论官内窘迫对新生儿脐血甲状腺激素水平有影响,脐血甲状腺激素可作为宫内窘迫是否得到有效纠正及胎儿宫内窘迫后新生儿窒息病情判断的参考指标。AIM To explore the changes and clinical significance of thyroid hormone in umbilical blood in fetal intrauterine asphyxia. METHODS The concentrations of thyroid hermones(T4 TSH)were tested by radioimmunoassay in umbilical venous blood after birth. They were divided into groups comparatively by if they were with intrauterine asphyxia history and intrauterine asphyxia was corrected. RESULTS Thyroid hormones(T4 TSH)in umbilical blood of the group which was with intrauterine asphyxia history and was corrected before birth and was no newborn asphyxia(Apgar score at 1 minute〉7)were no differences from the compared group. The group with intrauterine asphyxia history was longer lack of oxygen and was not corrected before birth was the newborn asphyxia group. Their T4 levels in umbilical blood decreased with the increasing degree of asphyxia and their TSH levels rised with the increasing degree of asphyxia. CONCLUSIONS Intrauterine asphyxia affected the concentrations of thyroid hormone in newborn umbilical blood. Thyroid hormone in the umbilicus blood can be regarded as a reference index of judging if intrauterine asphyxia has been effectively corrected and the illness judgment of newborn asphyxia after intrauterine asphyxia.
