摘  要:目的:观察左卡尼汀对糖尿病肾病患者糖脂代谢、血液流变学的影响。方法:68例糖尿病肾病患者随机分为2组,治疗组33例,对照组35例,两组均给予糖尿病常规治疗,治疗组加用左卡尼汀针剂治疗,疗程21 d,观察治疗前、后2组血糖、血脂、血液流变学指标的变化。结果:(1)两组治疗后的血糖、甘油三酯及胆固醇均比治疗前明显降低(P<0.01或0.05),且血糖、甘油三酯以治疗组下降更为显著,差异有统计学意义(P<0.01);(2)两组治疗后的全血比高切粘度、全血比低切粘度及治疗组的血浆比粘度均比治疗前显著降低(P<0.01),且全血比高切粘度以治疗组下降更为明显,差异有统计学意义(P<0.01)。结论:左卡尼汀能降低糖尿病肾病患者血糖、血脂水平,改善血液流变学。Objeetive: To observe the influence of L-carnitine on glucose and lipid metabolism and hemorheology in patients with diabetic nephropathy. Methods: Sixty-eight patients with diabetic nephropathy were randomly divided into two groups: control group (n = 35) only received conventional therapy for diabetes, and treatment group (n = 33) were treated with conventional therapy plus intravenous L-carnitine (2.0 g/d) for 21 days. Blood glucose, lipid and hemorheology were detected in all patients before and after therapy. Results: (1) After treatment, Levels of glucose, triglyceride and cholesterol, and high and low shear viscosity of whole blood, and specific viscosity of plasma showed an obvious decrease after therapy in both groups (P〈0.01or0.05). Glucose and triglyceride levels, and high shear viscosity were significantly lower in treatment group than those in control group (P〈0.01). Conclusion: L-carnitine can decrease the blood glucose and lipid, and improve the blood viscosity in patients with diabetic nephropathy.
