乳腺癌术后分段放射治疗对肺部的放射性放射性损伤

摘　　要：目的 探讨乳腺癌术后化疗同时射野分阶段放疗对放射性肺炎发生率的影响。方法 126例乳腺癌术后患者分为研究组与对照组，每组63例。研究组术后给予CAF方案连续化疗6周期，拆线后即行腋锁野常规分割照射，DT总量50Gy，完成照射后休息4周后续行胸壁野照射，方法剂量同腋锁野。对照组术后给予CAF方案先行化疗2周期，后行腋锁、胸壁野同时常规分割照射，DT总量50Gy，休息4周后追加4周期化疗。结果 研究组放射性肺炎发生率为7．9％，对照组为20．6％，差异有显著性（P〈0．05）。5年生存率研究组68．1％，对照组69．8％，差异无显著性（P〉0．05）。局部复发率、远处转移率差异无显著性（P〉0．05）。骨髓抑制均为Ⅰ～Ⅲ度白细胞下降，积极处理后未影响治疗。结论 乳腺癌术后化疗同时射野分阶段放疗可明显降低放射性肺炎发生率，并不提高局部复发率。Objective To discuss the effect of postoperative chemotherapy and radiotherapy to the incidence of radioactive pneumonia in breast cancer. Methods All 126 cases of postoperative breast cancer patients were divided into study group and control group,63 eases respectively. The study group was given CAF chemotherapy 6 eyeles continuously and radiotherapy from armpit field by stages after stitches removed. The total dose was 50Gy.After 4 weeks, the radiotherapy from chest field was given as above. The control group was given CAF chemotherapy 2 cycles continuously and radiotherapy from armpit and chest field under routine sehedule,50Gy total close. Four more chemotherapy was given at intervals of 4 weeks. Results The incidence of radioactive pneumonia in study group was 7.9% , while in control group was 20.6%. The difference was significant（ P 〈 0. 05）. The 5-year survival rate in the study group was 68.1% , while in the control group was 69.8 %. The difference was insignificant（ P 〉 0.05）. There was no significant difference about the incidence of the recurrence and remote metastatic between the two groups. The arrest of bone marrow in both groups was that WBC decreased by 1-3 grade, so therapy was not interrupted, Conclusion The incidence of radioactive pneumonia in breast cancer postoperative ehemotherapy can he decreased greatly by radiotherapy by trains,white the local recurrence rate is not elevated.
