摘　　要：目的：探讨急性生理学及慢性健康状况评分（APACHEⅡ）系统用于SICU护理管理的可行性及临床使用的价值，旨在全面提高危重病护理质量．使危重病管理更加科学化。 方法：自2001年起将APACHEⅡ评分系统应用于SICU护理管理中，即在病人入SICU后的24h．72h、一周，以后1／周进行APACHEⅡ评分，对APACHEⅡ≥12分的病人加强监护。按照危重病护理要求，结合科室实际情况，制定管理质量标准，建立阈值，评价护理质量。 结果：近5年来，通过使用该评分系统，有效减少了意外的发生．降低了气管插管率、降低了ICU病人精神障碍发生率．压疮发生率．避免了深静脉血栓的发生等，并提高了医生和病人的满意率。危重病护理的主要指标呈平稳上升趋势。 结论：在SICU护理管理中应用APACHEⅡ评分是一种科学的管理方法，有助于提高SICU护理质量。Objective： To evaluate the use of APACHE Ⅱ scoring system in a surgical intensive care unit by nurses, in order to improve the quality of nursing and the management of critically ill patients. Methods： From 2001, the APACHE Ⅱ scoring system was introduced to our SlCU, and all patients were evaluated by nurses at 24 h, 72 h, 1 week and every week thereafter after the admission to SlCU. Special nursing care was provided if the APACHEⅡ score was greater than 12. Results： After the introduction of the APACHEⅡ scoring system, the incidence of intubaUon, mental disorders, bedsore and deep venous thrombosis was decreased. The rate of satisfaction by doctors and patients was elevated. Conclusions： APACHE Ⅱ scoring system is useful in SlCU to improve nursing care and management of critically ill patients.
