体表电刺激治疗神经外科手术术后疼痛临床观察

摘　　要：目的:观察体表电刺激治疗神经外科手术术后疼痛的效果及副作用的发生率。方法:40例择期神经外科手术病人,随机分为二组:病人自控镇痛复合韩氏仪组(+HANS,n=20),病人自控镇痛组(-HANS,n=20)。所有病人气管拔管前予氢溴酸高乌甲素负荷量4mg静注,气管拔管后采用PCA镇痛。PCA使用药物:0.02%氢溴酸高乌甲素+0.02%甲氧氯普胺混合液100m l。其中(+HANS)组加用HANS治疗仪刺激一侧合谷-劳宫穴及患侧交感、神门透肾、外肺、脑干透皮质下等耳穴,刺激频率为2/100疏密波交替,间隔2h刺激1h。于术后24h内定时观察VAS镇痛评分,Ram e镇痛评分,镇痛药用量、副作用。结果:二组病人都能达到满意的镇痛效果,但+HANS组患者VAS评分明显少于-HANS组(P<0.01),镇痛剂用量明显减少(P<0.05),副作用发生率明显下降。结论:体表电刺激镇痛应用于神经外科手术术后疼痛可以达到增强镇痛、减少并发症之效应。Objective： To observe the effect of Han＇ s Acupoint Nerve Stimulator relieving pain after neurosurgery ope .ration and the side effect, Methods ： 40 patients who should be undergone neurosurgery are randomly divided into two groups. One group is self - controlled relieving pain combining HANS instrument （ ＋ HANS , n =20） . Another group is self-controlled relieving pain only （ - HANS , n =20） . All patients are infused lappaconitine loading dose 4mg into vein before trachea extubation and PCA are adopted to relieve pain after trachea extubation. PCA remedy： 0.02% lappaconitine plus 0.02% metoclopramide 100ml. ＋ HANS group are adopted HANS instrument stimulating one side hegu - laogong acupoint and jiaogan, shenmen penetrating shen, waifei, naogan penetrating pizhixia ear acupoints in the trouble side lh at 2h intervals, 2/100Hz electrical stimulation is used in this opereation. All patients are time - observed in 24h after operation. Then VAS score and Rame score are evaluated and the dosage of anesthetic are calculated, and the side effect are observed. Results ：Two groups patients all obtain satisfactory relieving pain effect. But ＋ HANS group average VAS score is lower than that in HANS group （ P 〈0.01 ）, and the dosage of anesthetic is reduced obviously, and the rate of side effect taking place is also decrease largely. Conclusion ： It can enhance the effect of relieving pain and reduce the side effect that applying anesthetic combining acupuncture therapy to neurosurgery pain after operation.
