抗房颤起搏器对缓慢心率患者合并阵发性房颤的预防作用

摘　　要：目的评价抗房颤起搏器对缓慢心率患者阵发性房颤(PAF)的预防效果。方法14例心动过缓并PAF患者,12例置入Vitatron900E型起搏器,2例置入St.JudeMedicalIntegrityTMAFXDR5346型起搏器,对患者进行一年随访,记录术后第1月(预防房颤程序未启动)、术后第2月、术后第6月、术后第12月房颤(AF)事件。结果患者术后2个月较术后1个月,术后6个月、12个月较术后第2个月有症状PAF数(9.21±5.47和16.18±9.82;4.31±3.22,3.89±2.87和9.21±5.47),PAF总数(24.31±27.24和46.23±31.52;9.34±8.37,9.16±8.25和24.31±27.24),AF总持续时间[(2.63±1.21)d和(3.18±1.58)d;(1.16±0.86)d,(1.02±0.82)d和(2.63±1.21)d],AF负荷[(8.77±4.57)%和(12.62±5.68)%;(3.86±2.52)%,(3.42±1.79)%和(8.77±4.57)%]均降低(P<0.05)。结论抗房颤起搏器能减低心率缓慢患者阵发性房颤的发生,降低房颤负荷,具有明显的短期及中期疗效。Objective To evaluate the effect of anti-atrial fibrillation cardic pacing on prevention of paroxysmal atrial fibrillation in patients with bradycardia. Methods 14 patients with bradycardia and paroxysmal atrial fibrillation were implanted Vitatron 900E pacemaker and 2 patients were implanted Integrity^TMAFxDR5346 pacemaker. PAF episodes were surveyed in the first month （the preventive pacing algorithms not be applied）, the second month , the sixth month , the twelfth month after being implanted. Results The number of symptomatic PAF episodes （9.21±5.47 vs.16.18±9.82. 4.31±3.22,3.89± 2.87 vs. 9.21±5.47）（all P〈0.05）, the number of PAF episodes stored （24.31±27.24 vs. 46.23±31.52. 9.34±8.37,9.16±8.25 vs. 24.31 ±27.24 ） （all P〈O.05 ）, AF total duration[ （2.63±1.21）d vs. （3.18±1.58）d. （ 1.16±0.86 ）d, （ 1.02±0.82 ）d vs. （2.63 ± 1.21 ）d ] （all P〈0.05 ）and AF burden [ （8.77±4.57）% vs. （12.62±5.68）%. （3.86±2.52）%, （ 3.42± 1.79）% vs. （8.77±4.57）%] （all P〈0.05 ）were significantly decreased in the second month than in the first month , and in the sixth month , in the twelfth month than in the second month after implanted. Conclusion Anti-atrial fibrillation cardic pacing can decrease occurrence of PAF episodes and AF burden in patients with bradycardia.
