危重心脏瓣膜病23例外科治疗及围手术期处理

摘　　要：目的探讨危重心脏瓣膜病的手术治疗和围术期处理。方法对23例危重心脏瓣膜病患者行手术治疗。术前心功能Ⅲ级16例,Ⅳ级7例。二尖瓣置换术11例,主动脉瓣置换术8例,二尖瓣加主动脉瓣置换术4例;同时行三尖瓣成形术12例,左房血栓清除术10例。结果早期主要并发症为室性心律失常、低心排血量综合征、多器官功能衰竭。早期死亡1例,存活22例。结论危重心脏瓣膜病的外科治疗应注重围术期处理,适当选择手术时机,合理纠正病变,加强术后并发症的防治。Objective To summarize the experience of surgical treatment and peri-operative handling for cases with critical valvular disease of the heart. Methods 23 patients with critical valvular disease of the heart, among them 16 cases with Ⅲ grade of heart function before operation, 7 cases with Ⅳ grade, underwent surgical treatment, Mitral valve replacement was performed on 11 cases, 8 cases suffered from aortic valve replacement, both mitral and aortic valve replacement were performed on 4 cases. Simultaneously, tricuspid valvuloplasty was implemented in 12 patients; left atrial thrombectomy was performed on 10 cases, Results The main complication in primary stage were ventricular arrhythrnia, low cardiac output syndrome and multiple organ failure. One case died in early stage, 22 patients were survival, Conclusions Surgical treatment for patients with critical alular disease of the heart should think highly of handling in peri-operative period, choose appropriate operative opportunity, correct pathological changes, strengthen the prevention and treatment of postoperative complications.
