摘 要:目的:探讨三维动态增强磁共振血管成像技术(3D-DCE MRA)显示脊髓Adamkiewicz动脉(AKA)的最佳扫描参数、扫描延迟时间及图像重建方法。方法:17例行胸、腰椎MRI检查,发现有脊髓、椎体或附件病变而需行MRI增强检查的病例,利用test bolus技术来预测对比剂注射延迟时间,采用3D-DCE MRA技术和各种图像后处理方法来显示脊髓AKA。结果:3D-DCE MRA的扫描参数为TR/TE 5.3/1.4,翻转角35°,扫描层厚1.4 mm,重建层厚0.7mm,K空间采用椭圆填充技术,FOV43cm×22cm,覆盖范围包括T5~L4的区域。MPR和MIP在显示脊髓AKA方面具有优势。结论:采用优化的扫描参数和恰当的图像重建技术,3D-DCE MRA能清晰显示脊髓AKA,可作为无创性血管成像的首选检查。Objective: Using three-dimensional dynamic contrast-enhanced magnetic resonance angiography (3D-DCE MRA) to visualize the artery of Adamkiewicz (AKA), and to investigate the optimized scanning parameters, scan delay time and image post-processing methods. Methods: 17 patients with lesions of the spinal cord or vertebrae underwent contrast-enhanced MR angiography of the thoracoabdominal aorta. Test bolus was used to predict the scan delay time, 3D-DCE MRA technique and various image post-processing methods were applied to visualize the artery of AKA. Results: The scanning parameters were as follows: TR/TE 5.3/1.4, flip angle 35° , slice thickness was 1.4 mm with reconstruction interval of 0.7 mm. The K-space was filled by using an elliptic-centric order. FOV 43cm × 22cm. Scanning coverage was from the level of T5 to L4. Muhiplannar projection reconstruction (MPR) and maximum intensity projection (MIP) were advantageous in depicting AKA. Conclusion: Using the optimized scanning and reconstruction techniques, 3D-DCE MRA is able to depict AKA clearly. As a noninvasive imaging technique, optimized 3D-DCE MRA should therefore be the method of choice for the depiction of AKA.
