摘  要:目的研究全身及局部应用心钠素对豚鼠耳蜗微循环的影响,为临床治疗耳蜗微循环紊乱所致疾病提供实验依据。方法30只豚鼠随机分为静脉用药组和外淋巴用药组及对照组。给药前后监测豚鼠耳蜗血流量(cochlear bloodflow,CoBF)和平均动脉血压(mean aterial blood pressure,MABP),同步输入计算机,进行统计分析。结果各对照组用药后MABP、CoBF在注射期和恢复期均保持相对稳定;不同静脉给药组MABP、CoBF时相变化的趋势不同,存在组间差别(P<0.05);外淋巴用药组MABP下降不明显,CoBF在开始给药后即开始上升,给药结束后仍高于基础值;各给药组与对照组比较,差异有统计学意义(P<0.05)。结论心钠素静脉用药虽然降低全身血压,但能有效提高耳蜗血流量,且不同浓度心钠素对耳蜗微循环作用机制可能存在差异。Objective The purpose of the current study was to assess the potential influence of atrial natriuretic peptides (ANP), a cardiac hormone which exerts natriuretic, diuretic and vasorelaxant effects ,on cochlear blood flow in guinea pigs so as to provide the experimental basis for the clinical drug use. Methods Guinea pigs were randomly divided into intravenous infusion (iv) group and perilymph application group. A control group was treated equivalently and infused with vehicle only. Baseline measurements of MABP and CoBF( monitored by laser doppler flowmeter) were established. Measurements were recorded during the infusion and for a recovery period. Results In saline group, CoBF and MABP remained stable for baseline, infusion and recovery period. In iv groups, results of repeated - measures ANOVA showed statistic significance( P 〈 0.05) for differences in CoBF and MABP across concentration. In perilymph application group, CoBF increased immediately and remained high above baseline while MABP remained stable. This was significantly different from changes in artificial perilympha group( P 〈 0. 05 ). Conclusion ANP can improve CoBF to various degree though MABP is decreased in a dose - dependent way. The mechanism that ANP affectes the cochlear microcirculation is different in case of different concentration.
