Analysis on the relationship between the low frenquency phase of sinusoidal harmonic acceleration test and canal paresis in unilateral semicanal paralysis

摘  要:目的探讨冷热试验一侧半规管功能麻痹(canalparesis,CP)与正弦谐波加速度试验(sinusoidalharmonicaccelerationtest,SHAT)低频相位的相关性。方法将2004年至2005年因眩晕而行冷热试验和正弦谐波加速度试验的120名患者根据CP值的大小分3组,所选对象均为一侧半规管麻痹。CP值在22%~40%之间的64名患者为A组,在41%~60%之间的33名患者为B组,>60%的23名患者为C组。对A、B、C三组的低频0.01、0.02Hz的相位参数进行统计,并分别与其CP值进行比较,观察它们之间的异常吻合率。结果C组相位值明显高于A、B两组,差异有显著性P<0.01,并且提示的异常吻合率最高。C组在两个频率上的异常吻合率分别为56.52%、34.78%,A组分别为9.37%、7.81%,B组分别为15.15%、15.15%,A组与B组相位比较无显著性差异(P>0.05)。结论冷热试验与SHAT试验都能反应低频状态下的眼动反射,但二者不一定完全吻合。异常吻合率与CP值的大小有关,随CP值增大,吻合率增高,且0.01Hz比0.02Hz敏感性更高。两种测试尚不能互相替代,联合应用可以提高诊断率。Objective The aim of this research was to explore the relationship between the low frequency phase of sinusoiclal harmonic acceleration test(SHAT) and canal paresis(CP) in unilateral sernicanal paralysis. Methods 120 patients were divided into three groups according to the values of CP. The patients with vertigo undertook the caloric test and SHAT (from 2004 to 2005). The function of one side of the semicircular canal of all the patients was low. For 64 patients in Group A, the values of CP were between 23% to 40%, for 33 patients in Group B, 41% to 60 %, and for 23 patients in Group C, the values of CP 〉 60%. The authors analyzed statistically the low frequency phase parameters of the three groups, comparing with the value of CP. For the abnormality coincidence among them, low frequency 0.01 Hz and 0.02 Hz were selected. Results The phase parameter of Group C was higher than Group A & B, showing the statistical difference, P 〈 0.01 ,and the abnormalities were most.The abnormality coincidence of the two frequency of Group C was 56.52% and 34.78%, Group A was 9.37% and 7.81%, Group B was 15.15% and 15.15%. There was no statistic difference between Group A and B,P〉0.05.Conclusion Both caloric test and SHAT could record the nystagmus, but the outcome of the abnormality coincidence was dissatisfactory. The abnormality coincidence had positive correlation with the value of CP, and frequency 0.01 Hz was more sensitive. Neither of any one of the two tests could replace each other and could be used jointly to provide more valuable clinical information.
