摘 要:目的探讨碱性成纤维细胞生长因子(basic fibroblast growth factor,bFGF)与凋亡调控基因Bax、Bcl-2在胎膜上的表达及与胎膜早破(premature rupture of membranes,PROM)发生的关系。方法采用免疫组化染色方法检测30例PROM及30例正常妊娠者的绒毛膜羊膜上bFGF与Bax、Bcl-2的表达。结果bFGF和Bax在滋养细胞、上皮细胞、间充质细胞和成纤维细胞胞浆内有表达,Bcl-2在绒毛膜合体滋养细胞上有表达;PROM组bFGF阳性平均积分光密度值(0.051±0.012)与正常组(0.098±0.027)比较明显降低(P<0.01),PROM组Bax阳性平均积分光密度值(0.086±0.021)与正常组(0.046±0.018)比较明显增高(P<0.01),PROM组Bcl-2阳性平均积分光密度值(0.058±0.020)与正常组(0.050±0.014)比较差异无统计学意义(P>0.05);PROM组bFGF与Bax呈负相关(r=-0.616,P<0.01),正常组bFGF与Bax间无相关性(r=-0.302,P>0.05)。结论绒毛膜羊膜上bFGF的低水平表达及过度细胞凋亡对PROM的发生有重要促进作用。Objective To investigate the expression of basic fibroblast growth factor(bFGF) and proapoptotic gene Bax, Bcl-2 in amniochorion and their relationship with premature rupture of membranes (PROM). Methods Immunohistochemical stain was used to detect the expression of bFGF, Bax and Bcl-2 in amniochorion collected from 30 pregnant women with PROM and 30 normal pregnant women without PROM. Results The expression of bFGF and Bax were located in the plasma of all types of cells including trophoblast, epithelial, mesenchymal and fibroblast, but the expression of Bcl-2 was only observed in the chorionic syncytiotrophoblast; the positive average integral calculus oculus debtor of bFGF in the PROM group was significantly lower than that of the normal group (0.051±0.012 vs 0. 098 ± 0. 027, P〈0. 01 ), and that of Bax was significantly higher than that of the control (0. 086±0. 021 vs 0. 046±0. 018, P〈0.01). However, the expression of Bcl-2 did not show any differences between the two groups (0. 058±0. 020 vs 0.050±0. 014, P〉0.05). Negative correlation was found between the bFGF and Bax expression in the PROM group (r= -0. 616, P〈 0.01) and no correlation was shown with the normal group (r= -0. 302, P〉0.05). Conclusions The reduced expressions of bFGF and excessive apoptosis in the amnio-chorion play an important role in PROM.
