摘 要:目的:探讨高血压病患者大动脉弹性功能的改变及其影响因素。方法:应用脉搏波速度自动测量系统对114例健康人和476例高血压患者进行颈动脉-股动脉脉搏波速度(carotid-femoralpulsewavevelocity,CFPWV)检测。结果:高血压患者的CFPWV显著高于健康人(P<0·01)。年龄是影响健康人大动脉弹性功能最主要的因素;年龄、收缩压和心率是与高血压患者的CFPWV关系最密切的参数。结论:增龄导致大动脉弹性功能减退,高血压加速这一过程;脉搏波速度检测为临床提供一种无创、简便的评估高血压患者血管功能状态的方法。Objective:To explore the change of large arterial elasticity in essential hypertensive patients and the influencing factors. Method: Automatic pulse wave velocity measuring system was applied to examine carotidfemoral pulse wave velocity (CFPWV) as an index reflecting elasticity of large arteries, ll4 healthy subjects and 476 hypertensive patients were recruited in the study. Result: CFPWV was significantly higher in the hypertensive patients group than healthy subjects group (P 〈 0.01 ). CFPWV increased with age in both groups (P 〈 0.01 ). A stepwise multiple regressive analysis demonstrated that age was significantly associated with CFPWV in healthy subjects. Age, systolic blood pressure and heart rate were positively and independently related to CFPWV in hypertensive patients. Conclusion: Advanced age contributes to the degeneration of the large arterial elasticity, high blood pressure accelerates the process. Age, systolic blood pressure and heart rate have close relationship with large arterial elasticity of hypertensive patients. PWV could be used as a noninvasive, simple measurement to evaluate large arterial elasticity of hypertensive patients.
