摘 要:目的:探讨米非司酮对母胎界面Th1/Th2型细胞因子动态平衡的影响。方法:将63例早孕期妇女随机分为2组,一组一次服用米非司酮200 mg,另一组为对照组,收集其蜕膜组织。应用免疫组化法,评价Th1型细胞因子(IL-2、IFN-γ)、Th2型细胞因子(TGF-β2、IL-4)的表达。结果:正常妊娠时,在母-胎界面Th2型细胞因子(IL-4)以及TGF-β2的表达较高;Th1型细胞因子(IL-2、IFN-γ)的表达较低,尤其是IL-2。服用米非司酮后,蜕膜Th1型细胞因子(IL-2、IFN-γ)表达显著升高;而母-胎界面Th2型细胞因子(IL-4)以及TGF-β2的表达无明显变化。结论:米非司酮打破了正常妊娠时母胎界面Th2型免疫优势;显著升高Th1型细胞因子(IL-2、IFN-γ)表达,形成了Th1型免疫偏离,导致流产的发生。Objective :To probe into effects of Mifepristone on the Th1/Th2 balance at materno-fetal interface. Methods:Sixty three healthy women in early pregnancy were randomized into two groups, 30 women used mifepristone 200 mg, and 33 women as control. Their decidual tissue was collected by curettage in 48 hours of taking the drug. The Th1/Th2 cytokine expressions in decidua were analyzed by immunohistochemistry, Results:In the decidual tissue of normal early pregnancy, there were significantly higher densities of Th2 cytokines, IL-4 and TGF-β2, and lower density of Th1-type cytokine, IFN-γ and IL-2. After having treated by Mifepristone, the decidual tissue appeared to significantly higher densities of Th1-type cytokines (IL-2, IFN-γ). Conclusion: Our findings suggest that Mifepristone can up-regulate expression of the Th1-type cytokines, and cause a shift towards Th1 bias, which leads to abortion.
