包皮横形岛状皮瓣一期成形术治疗尿道下裂(附14例报告)

摘　　要：目的探讨包皮横形岛状皮瓣一期成形术（Duckett术式）治疗尿道下裂的效果。方法1997年4月～2003年4月采用Duckett术式一期尿道成形术修复尿道下裂14例，其中阴茎冠状沟型3例，阴茎体型9例，阴茎阴囊型2例。结果一次性手术成功11例（78％）。手术后出现尿瘘3例（21％），其中2例于手术6个月后行尿瘘手术修补成功，1例于手术后2周，尿瘘自愈。无1例出现尿道狭窄。随访9例，时间6～24个月，无1例排尿异常。结论包皮横形岛状皮瓣一期成形术（Duckett术式）是治疗尿道下裂的一种基本较好的手术方法。Objective To study the effect of one-stage urethroplasty with transverse preputial island flap （Duckett＇s procedure）performed for hypospadias. Methods From April 1997 to April 2003,14 cases of hypospadias were treated by transverse preputial island flap urethroplasty. The age of the patients ranged from 2 to 28 years,mean 9 years. In 14 cases, 3 were glans and subcoronal type,9 penile type,2 penoscrotal type. Results The operation was successful in 11 patients（78%）. Urethral fistula occurred in 3 patients（21% ）. 2 patients who developed urethral fistula were repaired successfully six month after operation. One who developed urethral fistula was self-healed two weeks after operation. No urethral stricture occurred in all cases. 9 patients were followed up for 6-24 months with satisfactory result. Conclusion Duckett＇s procedure is a fairly advisable technique for hypospadias.
