摘　　要：目的对比分析H IV抗体筛查试验阳性结果与免疫印迹试验(WB)结果,探求H IV抗体筛查阳性样本ELISA的S/CO值与确认实验结果的关系。方法对952份筛查阳性样本的ELISA结果与WB确认结果进行比较,计算ELISA的敏感性、特异性,并依据ELISA检测S/CO值变化探求与WB结果的关系。结果ELISA与WB的阳性符合率为93.39%,ELISA检测H IV抗体的敏感性为99.53%,特异性为96.49%;确认为阳性标本的S/CO平均值为18.38,gp160、gp120、gp41、p66、p51、p31、p24条带出现的百分率都在93%以上,其中gp160出现的百分率为100%;确认阴性与不确定标本S/CO平均值分别为3.03与2.22,经统计学分析,两组之间无显著性差异。结论筛查试验存在假阳性结果,H IV抗体结果的报告必须以确认结果为准。高S/CO值(≥6.0)预示H IV抗体阳性的可能性较大,而S/CO值较低时H IV抗体阴性及不确定的可能性较大。WB确认方法在区分阴性与不确定标本中存在不足。Objective To analyze the relationship between S/CO value of ELISA and WB results by comparing the HIV antibody screening positive results and Western Blot test results. Methods ELISA results of 952 screening positive samples were compared with WB results, the sensitivity and specificity of ELISA were calculated,the relationship between ELISA and WB results were studied according to the variations of ELISA S/CO value. Results The positive consistency of ELISA and WB was 93.39%, and the sensitivity of ELISA was 99.53% ,and the specificity was 96. 49%, S/CO average value of confirmed positive samples was 18.38,The appearance rate of gp160,gp120 ,gp41 ,p66,p51 ,p31 ,p24 reactive bands was over 93% ,that of gp160 was 100%. S/CO average value of confirmed negative and indeterminate samples were 3.03 and 2. 22 respectively, there was no significantly different between them. Conclusion The final HIV antibody test results must be confirmed by WB test as several false positives do exist in the screening test. When the S/CO value is high（≥6. 0） ,it is most possible that the sample is positive. WB test has limitation in differting “negative” from“indeterminate” samples.
