摘 要:目的:了解临床细菌性尿道感染患者细菌感染的流行趋势并对其药物敏感趋势进行监测,为临床合理用药提供依据。方法:通过对东方医院2001年1月1日至2005年12月31日临床细菌性尿道感染患者的标本中分离出1347株病原菌,进行调查分析和对其药物敏感趋势进行监测。结果:临床患者的标本中共分离出1347株细菌。主要致病菌依次为大肠埃希氏菌608株(45.14%)、粪肠球菌91株(6.76%)、肺炎克雷伯菌80株(5.94%)、屎肠球菌70株(5.20%),表皮葡萄球菌55株(4.08%)、铜绿假单胞菌54株(4.01%)。超广谱β-内酰胺酶(ESBL)确证试验结果:527株大肠埃希菌,产酶率34%;75株肺炎克雷伯菌,产酶率32%。53株表皮葡萄球菌,耐甲氧西林的表皮葡萄球菌(MRSE)产酶率15%;53株表皮葡萄球菌,β内-酰氨酶产酶率94%;89株粪肠球菌,庆大霉素高水平耐药株(HGAR)22%;链霉素高水平耐药株(HSAR)35%;耐万古霉素的肠球菌(VRE)6%;56株屎肠球菌,HGAR23%;HSAR48%;VRE9%。结论:细菌耐药严重,应根据细菌检验结果合理用药。Objective: Analysis of 1 347 strains of bacteria isolated from patients with bacterial infection and monitoring of their drug susceptibility, to provide basis for reasonable use of drugs in clinical practice. Methods: From 01/01/2001 to 12/31/2005, 12/31/2005,we collected 1347 strains of bacteria isolated from patients with bacterial urethral infection.The bacteria were cultured with blood dish and identified and monitoring of their drug susceptibility with the Vitek-32 manufactured by Biomerieux Company. Results:The 1347 strains of bacteria isolated from patients with bacterial urethral infection consisted of 608 strains (45.14%) of Escherichia coli, 91 strains (6.76%) of Enterococcus faecalis, 80 strains (5.94%) of Klebsiella genus, 70 strains (5.20%) of E.faecium,55 strains (4.08%) of Staphylococcus epidermidis, 54 strains (4.01%) of Pseudomonas aeruginosa. The incidence of ESBLs in 527 strains E coli and 75 strains K. pneumoniae was 34% and 32%, respectively. The incidence of MRSE in 53 strains Staphylococcus epidermidis was 15%.The incidence of β-lactamases in 53 strains Staphylococcus epidermidis was 94%.The incidence of HGAR in 89 strains E.faecalis and 56 strains E.faecium was 22% and 23%, respectively. The incidence of HSAR in 89 strains Enterococcus faecalis and 56 strains E.faecium was 35% and 48%, respectively.The incidence of VRE in 89 strains E, faecalis and 56 strains strains Efaecium was 6% and 9%, respectively. Conclusions: Our data may have great significance in the empirical use of antimicrobial agents for the treatment of infections.
