122例单纯性老年瘙痒症相关因素的病例对照研究

摘　　要：目的探索老年瘙痒症(SP)的相关因素。方法利用健康查体资料作描述性流行病学横断面研究,采用1:1病例对照研究的方法调查SP发病风险因子。选择2003年1月至2004年12月在北京医院健康查体的SP患者122例,选取未患SP的老年人作为对照,使用单因素和多因素条件logistic回归模型分析。结果1236例老年查体对象中,SP 827例,占所有研究对象67％,其中60-69岁95例(占SP 11．5％),70-79岁281例(占SP 34％),80-89岁320例(占SP 38．7%),90岁以上131例(占SP 15．8％)。多因素条件logistic回归分析结果显示:年龄、皮肤干燥、便秘是SP的危险因素(P<0．05,OR值分别为1．201,2.995,7.372);而每天适量补充维生素C、E和复合维生素B等是保护因素(P<0．05,OR值为0．068)。结论SP患病率随年龄增长而增加。除年龄外,对上述因素的干预有利于老年人群SP的防治。Objective To explore the risk factors of simple senile pruritus among the elderly in Beijing. Methods Using healthy specimens to study risk factors of senile pruritus among the elderly, 1：1 case control study was used to this investigation. Cases involved 122 patients with simple senile pruritus while the controls were 122 normal people who underwent physical check-up in Beijing hospital from January,2003 to December,2004. Univariate and multivariate conditional logistic regressions were used to analyze the database. Results Among all the 1236 investigated senior specimens,827,or 67% were senile pruritus patients,among which 95 aged 60 to 69 （11.5% of SP）,281 aged 70 to 79 （34% of SP）,320 aged 80 to 89（38.7% of SP） ,and 131 aged over 90（ 15.8% of SP）. Resuhs of the multivariate conditional logistic regression analysis showed aging, dry skin and coprostasis are major risk factors of simple senile pruritus （ P〈0.05, OR value are 1.201,2. 995,7. 372 respectivly）, while taking vitamins including Vits C,E and Bco was protective factors （P〈0.05, OR value was 0.068 ）. Conclusion Incidence of senile pruritus increased parallel to age. It was clear and rational that intervention and modification of those risk factors （except for age） and using protection factor could lead to a decrease in simple senile pruritus events in elderly population of Beijing.
