阻塞性睡眠呼吸暂停低通气综合征患者上气道扩张肌肌电活性的研究

摘　　要：目的:观察阻塞性睡眠呼吸暂停低通气综合征(OSAHS)患者上气道扩张肌肌电图(EMG)的变化,探讨其在OSAHS中的作用及意义。方法:对中、重度OSAHS患者及正常对照组分别行包括颏舌肌、腭帆提肌、腭帆张肌等上气道扩张肌肌电图检测并进行比较。结果:①OSAHS患者上气道扩张肌肌电活动均较正常人高(P<0.01)。②腭帆提肌、腭帆张肌肌电活动在OSAHS中、重度组间无显著性差异(P>0.05),OSAHS重度组的颏舌肌则明显高于中度组(P<0.01)。③同组内腭帆提肌、腭帆张肌肌电活动之间无显著性差异(P>0.05),而颏舌肌则显著高于前两者(P<0.01)。结论:在清醒状态下,OSAHS患者上气道扩张肌肌电活动代偿性升高,以维持气道的开放,颏舌肌是上气道开放的主要肌肉。Objective ：To observe the changes of the electronic muscle graph （EMG） of the dilatation muscles of upper airway in patients with obstructive sleep apnea-hyponea syndrome（OSAHS） and to explore their functions and significance. Methods： The myoelectronic activities of the levator palatini muscle, tensor palatini muscle, genioglosus muscle were measured by EMG in patients with moderate to serious OSAHS and were compared with the normal control. Results： ①The myoeletronic activities of dilation muscles of the upper airway in OSAHS patients were higher than those in the control（P 〈 0.01 ）. ②The myoeletronic activities of levator palatini muscle, tensor palatini muscle had no significance between the moderate OSAHS and severe OSAHS （P 〉 0.05 ）, but the myoeletronic activity of genioglosus muscle was higher than that in the moderate OSAHS group （P 〈0.01 ）. ③In the same group, the myo- eletronic activity had no significance between levator palatini muscle and tensor palatini muscle （P 〉 0.05） , while which in the genioglosus muscle was higher than those in the former two（ P 〈0.01 ）. Conclusion：The myoeletronic activity of dilatation muscles of upper airway increases compensatively in patients with OSAHS, while the genioglosus muscle plays a major role in the dilation of the upper airway.
