杏丁联合胞二磷胆碱治疗中老年椎-基底动脉供血不足性眩晕的研究

摘　　要：目的观察杏丁联合胞二磷胆碱治疗中老年人椎-基底动脉供血不足性眩晕的疗效,以及对患者血脂和血液流变学的影响。方法选择中年椎-基底动脉供血不足性眩晕急性发作期患者90例,随机分为2组,治疗组给予胞二磷胆碱0.75 mg、杏丁20 mL分别加入生理盐水250 mL中静滴,1次/d;对照组给予丹参注射液20 mL加生理盐水250 mL静滴,1次/d。2组均连用14 d,记录2组治疗效果和治疗前后血脂、血液流变学和经颅多普勒超声椎基底动脉血流速度及临床症状改善情况。结果治疗组眩晕、恶心呕吐等症状消失时间明显短于对照组(P<0.05),有效率(95%)明显高于对照组(73%)(P<0.05),且治疗后总胆固醇(TC)、三酰甘油(TG)、血黏度、血浆比黏度、红细胞聚集指数及红细胞变形能力均有明显改善(P<0.05),收缩期和舒张期末血流速度明显加快(P<0.01)。结论胞二磷胆碱联合杏丁可有效控制椎-基底动脉供血不足的急性发作,而且对血液流变学、血脂和椎-基底动脉血流速度也有明显改善作用。Objective It is to observe the curative effcct of Xingding injection combined with Citicolinc on vertigo induccd by insufficient blood-supply of vertebral-basilar artery and its effect on blood fat and hcmorheology in middle and old age patients. Methods 90 cases of patients with vertigo induced by insufficient blood-supply of vcrtcbral-basilar artery in acute onset stage were chosen and divided into two groups. Treatment group was given Citicolinc 0.75 mg ＋ Xingding 20 mL ＋ 250 mL normal saline by intravenous drip once a day. Control group was given Danshen injection 20 mL ＋ normal saline 250 mL by intravenous drip once a day. Both the groups were treated for 14 days. Curative effect and the changes of blood fat, hemorheology, blood flow speed of vertebral-basilar artery detected by TCD, and improvement of clinical symptoms were recorded before and after treatment. Results Disappearing time of symptoms such as vertigo, nausea and vomit was shorter and effective rate was higher in treatment group than those in control group （P 〈 0.05）. TC, TG and indexes of hemorheology were improved obviously （P 〈 0.05）, and blood flow speed in systolic and diastolic phase was faster in treatment group （P〈0.01）. Conclusion Xingding injection combined with Citicoline can effectively inhibit the acute onset of insufficient blood-supply of vertebral-basilar artery, and has a obvious improvement function on hcmorheology, blood fat and blood flow speed of vertebral-basilar artery.
