摘 要:目的探讨卡托普利对实验性急性胰腺炎的保护作用。方法63只SD大鼠随机分对照组、急性胰腺炎组、卡托普利干预组3组进行不同处理。对照组仅作胃窦切开再缝合术;急性胰腺炎组用十二指肠闭襻法制作急性胰腺炎模型;卡托普利干预组在胰腺炎模型制备后立即腹腔注射卡托普利针(5mg/kg)。在病程不同时点测定血浆肾素活性(PRA)、血管紧张素II(AngII)、内皮素(ET)、一氧化氮(NO)水平,并观察胰腺组织病理学改变。结果急性胰腺炎组随着病变的进展,胰腺炎病理由水肿向出血坏死发展,病程10h内,PRA、AngII以及ET与NO同步上升,ET/NO比值尚能维持相对平衡,胰腺炎病理主要表现为水肿性;但病程10h后,AngII、ET继续升高,PRA也维持极高水平,而NO则不再继续上升,致病程24hET/NO比值显著升高,胰腺炎病理表现为出血坏死性,胰腺病理学评分显著高于10h(P<0.05)。卡托普利干预组则胰腺炎病理改变减轻,胰腺组织病理学评分降低,且血浆PRA、AngII、ET水平低于急性胰腺炎组,而NO总体水平与急性胰腺炎组比较无显著降低,ET/NO比值基本与对照组一致。结论早期应用卡托普利对大鼠急性胰腺炎病变可能有保护作用。Objective To study the protective effect of captopril on experimental acute pancreatitis. Methods 63 SD rats were divided into three groups randomly: the control group, acute pancreatitis group and captopril intervention group, only gastrotomy was done in control group, acute pancreatitis was induced by closed duodenal loop techn/quein in acute pancreatitis group; and captop61(5mg/Kg) was injected i. p. at the beginning of acute pancreat/tis induced in captopril group; plasma renin activity(PRA), angiotension Ⅱ (AngⅡ), endothelin ( ET ) and nitric oxide (NO) levels were measured and the tissue sections were examined histopathologically in different time of the disease development. Results In acute pancreatitis group, histopathology revealed that pancreatitis developed from edematous to bleeding; in acute pancreatitis group, plasma renin activity, angiotesnion Ⅱ, endothelin, nitric oxide level increased as acute pancreatitis developed , but after 10 hours of the disease course, endothelin produced more and nitric oxide less. however, in captopril intervention group, angiotension Ⅱ level decreased and endothelin / nitric oxide ratio was similar to that of control group, the histopathologic score of pancreatitis was decreased significantly and the tissue damage was lessened. Conclusion Early use of captopril could has protective effect on acute pancreatitis in rats.
