摘 要:目的观察胰岛素对大鼠急性脊髓损伤后血管内皮生长因子(VEGF)表达及微血管再生的影响,探讨胰岛素在改善脊髓损伤微循环方面可能的作用。方法60只SD大鼠随机分为损伤对照组和胰岛素治疗组,建立脊髓损伤(SC I)模型;术后采用斜板试验和改良的Tarlov评分对大鼠急性脊髓损伤后的功能评估;采用免疫组织化学(S-P法)观察脊髓VEGF、微血管密度(MVD)的表达变化,并进行比较分析。结果术后各时相斜板试验和改良的Tarlov评分,胰岛素治疗组明显高于对照组;胰岛素治疗组各时相点VEGF的阳性表达均明显高于对照组;伤后14天,胰岛素组MVD计数明显增多,毛细血管密度增加,两组差异有统计学意义(P<0.05,P<0.01)。结论胰岛素可增强脊髓损伤后VEGF的表达,促进微血管再生,改善微循环。Objective To study the effect of insulin on the expression of VEGF and regeneration of blood vessel after acute spinal cord injury, and to study the possible effect of insulin in ameliorating microcirculation. Methods Sixty SD rats were divided into 2 groups, the control group and the insulin group. The animal SCI model was established with Allen's method. Inclined plane test and modified Tarlov Score were adopted to assess function of rats after acute spinal cord injury. The expression of VEGF and MVD were tested with immunocytochemistry technique, Results The outcomes by inclined plane test and modified Tarlov Score were higher in the insulin groups than that of the control group. Immunohistochemical staining revealed that the expression of VEGF was higher in the insulin group than that of the control group, and the counted number of MVD increase obviously in the insulin group( P 〈 0.05 or P 〈 0.01 ). Conclusion Insulin can reinforce the expression of VEGF after SCI, enhance the regeneration of capillary vessel,and ameliorate microcirculation.
