糖皮质激素对原发性肾病综合征患儿血清细胞因子的影响

摘　　要：目的探讨糖皮质激素(激素)对原发性肾病综合征(PNS)患儿血清IL-1-、6、转化生长因子-β1(TGF-β1)和TNF-α的影响。方法将PNS患儿25例分为激素治疗前和治疗后2组,比较3种不同病理类型血清细胞因子水平,采用酶联免疫吸附双抗体夹心法(ELISA)检测其血清IL-1-、6、TGF-β1及TNF-α水平。结果激素治疗前后PNS患儿血清IL-1-、6、TGF-β1、TNF-α水平差异有显著性(P<0.01)。微小病变型肾病(MCD)、膜性增生性肾小球肾炎(MsPGN)、局灶性节段性肾小球硬化(FSGS)3种不同病理类型血清IL-1-、6、TNF-α水平治疗前后差异无显著性(P>0.05);MCD治疗前后血清TGF-β1水平明显低于MsPNG、FSGS(P<0.01),治疗前MsPNG血清TGF-β1水平与FSGS差异无显著性(P>0.05),治疗后FSGS血清TGF-β1水平高于MsP-NG(P<0.01)。结论激素对PNS患儿血清多种细胞因子有抑制作用;不同病理类型PNS TGF-β1表达水平不同;FSGS表达最高,对激素治疗反应最差。Objective To study the effect of glucocorticoid on serum interleukin（IL）-1,IL-6,transforming growth factor-β1（TGF-β1） and tumor necrosis factor alpha （TNF-α） in children with primary nephrotic syndrome（PNS）.Methods The levels of cytokines on serum were compared in 3 different patho-type ,and IL-1,IL-6,TGF-β1 and TNF-α in serum were detected using enzyme-linked immunosorbent assay（ELISA） in 25 children with PNS before and after treatment with glucocorticoids.Results Significant difference of IL-1,IL-6,and TNF-α were found among the patho-types of minimal change nephritic disease（MCD）,mesangial proliferative glomerulonephritis（MsPNG） and focal segmental glomerulosclerosis（FSGS）（P〉0.05）.The level of TGF-β1 in MCD was lower than that in MsPNG,FSGS before and after the treatment（P〈0.01）,but there were no difference between MsPNG and FSGS before the treatment（P〉0.05）,and the level of it is FSGS was higher than that in MsPNG after the treatment（P〈0.01）.Conclusions It is suggested that glucocorticoid might inhibit serum IL-1,IL-6,TGF-β1 and TNF-α in children with PNS.Different patho-types of PNS have different levels of TGF-β1,and it is the highest in FSGS.Therefore,therapeutic reaction for glucocorticoid is insensitive.
