慢性乙型肝炎病毒携带儿童血清及外周血单个核细胞培养上清干扰素γ和白细胞介素-10的水平及其意义

摘　　要：目的探讨慢性乙型肝炎病毒(HBV)携带儿童血清及外周血单个核细胞(PBMC)经乙型肝炎病毒核心抗原(HBcAg)刺激后培养上清干扰素γ(IFN-γ)和IL-10水平及临床意义。方法HBeAg阳性慢性HBV携带儿童21例(A组)、抗-HBe阳性慢性HBV携带儿童20例(B组)及正常儿童19例(C组),每例采集外周静脉血5 mL,分离血清及PBMC,PBMC体外单独培养或与HBcAg共培养72 h后,检测血清及培养上清IFN-γ和IL-10水平。结果血清IFN-γ在各组间无明显差异,A和B组血清IL-10较C组显著增高。经HBcAg处理后PBMC培养上清中,IFN-γ水平在B和C组较A组显著增高。同时,经HBcAg处理后A和B组PBMC培养上清中IL-10水平均较C组显著增高,尤以A组明显。结论IL-10水平及PBMC对HBcAg应答低下在儿童HBV携带者发病机制中起一定作用。Objective To explore the levels of interferon γ（IFN-γ） and interleukin- 10（IL- 10） in sera and in hepatitis B virus （HBV） core antigen（HBcAg） stimulated culture supematants of peripheral blood mononuclear cells（PBMC） from chronic HBV carriers children and their clinical significance. Methods Five milliliter peripheral blood was collected from 21 HBeAg - positive children（group A） ,20 HBeAg- negative children（group B） and 19 normal controls（group C） ,sera and PBMC was isolated,after 72 h culture of PBMC with HBcAg or not. The levels of IFN - γ and IL - 10 in sera and in HBcAg stimulated or not stimulated culture supernatants of PBMC were detected. Results The level of sera IFN - γ had no difference between each group; the sera level of IL - 10 in group A and B were significantly higher than that of group C. The level of IFN - γ in HBeAg stimulated culture supernatants of PBMC from group B and C were higher than that from group A; while IL- 10 levels in HBeAg stimulated culture supematants from group A and B were higher than that from group C. Conclusions The level of IL - 10 and the lower reactivity to HBeAg of PBMC play roles in the mechanisms of chronic HBV carriers children.
