胰腺MRI:动态增强扫描最佳延迟时间窗的研究

摘  要:目的构筑正常胰腺时间-增强指数曲线,确定胰腺动态增强MRI最佳延迟扫描时间窗。方法60例非胰腺疾病患者,随机分为A、B、C3组各20例,用小剂量团注法测对比剂达腹主动脉峰值时间,按公式Delay=TV-A-1/4TA+t,使3组分别于t为0s、5s、10s行多时相动态增强MRI扫描。测量胰腺、肝脏及胰周血管的信号强度,计算增强指数,并对图像质量进行评分。结果B组在对比剂达腹主动脉后延迟5s扫描,胰腺实质增强指数为18.6,明显高于A组10.4和C组15.0(P<0.05),同时胰周动脉强化程度良好;延迟33s扫描,肝脏及胰周静脉增强指数分别为14.3和21.0,高于A组的10.7和15.8(P<0.05),而与C组的肝实质13.1无统计学差异(P>0.05)。结论在对比剂到达腹主动脉后延迟5s和33s左右行屏气扫描,是获得高质量胰腺MRI双期动态增强扫描的实用技术。Objective To make up the time versus enhancement index curves of normal pancreas and to investigate the best multiphase delay scanning time window . Methods 6 0 patients without pancreatic diseases were randomly assigned to A , B , C group . Test - bolus imaging was performed to determine aortic transit time (TV -A ) in individual patients. According to the formula: D = TVA - 1/4TA + t, the multiphase scanning time after administration of Gd - DTPA was defined at t = 0 , 5 , 1 0 seconds respectively to A , B , C group. Signal intensity of pancreas , liver , and per - pancreatic vessels were measured , enhancement index was calculated and images quality was assessed . Results B delay of 5 s after arrival of Gd - DTPA to the abdominal aorta , the enhancement index of pancreas in B group was 18.6 , which evidently higher than A group 10.4 and C group 15.0 (P 〈 O. 05 ) , meanwhile the per- pancreatic arteries were shown well. After a delay of 33s, the enhancement index of liver and per - pancreatic vein were 14.3 and 21.0 respectively which higher than that of A group ( 10.7 and 15.8 , P 〈 0.05 ) and had no statistically significant difference with C group in liver ( 13.1, P 〉 0. 05 ) . Conclusion Biphasic imaging at 5 and 33 seconds after arrival of contrast media to abdominal aorta is a practical method for acquisition of high - quality dynamic enhanced MR imaging of the pancreas.
