皮质下动脉硬化性脑病患者血浆抵抗素、瘦素水平及其相关性研究

摘　　要：目的探讨皮质下动脉硬化性脑病(SAE)患者血浆抵抗素、瘦素水平,及其与年龄、血压、血糖、血脂、胰岛素的关系。方法选择63例SAE患者和57例健康老年人,采用酶联免疫法测定2组空腹血浆抵抗素、瘦素水平,同时检测空腹血糖、血脂、胰岛素及测量血压,并进行2组比较。结果与对照组比较,SAE组抵抗素、瘦素、总胆固醇(TC)、胰岛素、收缩压(SBP)、舒张压(DBP)均明显升高,差异有显著意义(P<0.05或P<0.001),血糖、甘油三酯(TG)、高密度脂蛋白(HDL)、低密度脂蛋白(LDL)差异无显著意义(P>0.05)。SAE组抵抗素与SBP、TC、胰岛素均呈显著正相关(P<0.05或P<0.01),与年龄、DBP、血糖、TG、LDL、HDL均无相关性;瘦素与SBP、DBP、胰岛素均呈显著正相关(P<0.05或<0.01),与HDL呈显著负相关(P<0.05),与年龄、血糖、TC、TG、LDL均无相关性;抵抗素和瘦素呈显著正相关(P<0.05)。结论皮质下动脉硬化性脑病患者抵抗素、瘦素水平升高,它们可能通过联合影响机体能量代谢和平衡参与皮质下动脉硬化性脑病发生。Objective To explore the level of serum resistin and leptin and the relationship to age, blood pressure, glucose , blood lipid and insulin in the patients with subcortical arteriosclerotic encephalopathy （SAE）. Method 63 patients with SAE and 57 healthy elderly were selected. Fasting serum resistin level and leptin level were measured by ELISA in the 2 groups. Meantime, fasting glucose, blood lipid, insulin level and blood pressure were measured and then compared. Results Serum resistin level, leptin level, total cholesterol （TC）, insulin, systolic blood pressure （SBP） and diastolic blood pressure （DBP） in the SAE group were significantly highter than those in the normal control group （ P 〈 0.05 or P 〈 0.01）, But there was no signifcant difference in glucose,triglyceride （TG） ,high density lipoprotein （HDL） and low density lipoprotein （LDL） （ P 〉 0.05）. In group SAE, serum resistin level was positively correlated with SBP, TC and insulin （ P 〈 0.05 or P 〈 0.01 ）, and not correlated with age, DBP, glucose, TG, LDL and HDL. Leptin level was positively correlated with SBP, DBP and insulin （ P 〈 0.05 or P 〈 0.01 ）, significantly negatively correlated with HDL （ P 〈 0.05 ）, and not correlated with age, glucose, TC, TG, and LDL. Serum resistin level was positively correlated with leptin level （ P 〈 0.05）. Conclusion The serum resistin and leptin levels in the patients with SAE elevate and they may play regulative roles in the development of SAE by interfering energy metabolism and balance.
