老年稳定型心绞痛患者外周血中内皮祖细胞变化及其影响因素

摘　　要：目的观察老年稳定型心绞痛(SA)患者外周血中内皮祖细胞(EPC s)数量及功能的变化,初步探讨心血管危险因素对其影响。方法选择老年SA患者和年龄匹配的对照者各20例,用密度梯度离心法从外周血中分离单个核细胞,用流式细胞仪测量外周血中CD+34细胞以鉴定EPC s,并进一步用倒置荧光显微镜鉴定F ITC标记荆豆凝集素Ⅰ(F ITC-UEAⅠ-)和D iI标记的乙酰化低密度脂蛋白(D iLDL)双染色阳性细胞为正在分化的EPC s。采用黏附能力测定试验、改良Boyden小室和体外血管生成试剂盒分别观察EPC s的黏附能力、迁移能力和体外生成血管能力。结果老年SA患者外周血中EPC s数量明显低于对照组,其黏附能力、迁移能力和体外生成血管能力也明显受损。SA组患者EPC s数量与收缩压、空腹血糖水平呈负相关。结论老年SA患者外周血中EPC s数量降低且功能减退,其数量降低与高血压、糖尿病有关。[Objective] To investigate the changes in the number and funcional activity of endothelial progenitor cells （EPCs） from peripheral blood in patients with stable angina pectoris （SA） and the influence of cardiovascular risk factors on it. [Methods] Mononuclear cells were isolated by density-gradient centrifugation from 20 old patients with stable angina pectoris and 20 age-matched controls. Immediately after isolation, mononuclear cells were plated on 24-well culture dishes coated with human flbronectin. Flow cytometry was used to identify EPCs. In addition, dual-stained cells positive for both FITC-UEA- Ⅰ and DiLDL were judged to be EPCs, and they were counted. EPCs adhesive, migratory and vasculogenesis capacity were assayed by adhesion assay, modified Boyden chamber and In Vitro Angiogenesis Assay. [Results] Human EPCs from old patients with stable angina pectoris exhibit decreased numbers and impaired adhesion, migratory and incorporation into vascular structures. The number of EPCs from SA group inversely correlated with systolic pressure and fasting plasma glucose level. [Conclusions] The old patients with stable angina pectoris have decreased numbers of circulating EPCs with impaired functional activity. The decreased number correlates with hypertension and diabetes.
