普通电极经右颈内静脉床旁临时起搏的应用

摘　　要：目的:探讨使用普通电极经右颈内静脉在床旁快速、安全安置心脏临时起搏器方法。方法:对12例病例在体表心电图指导下,经右颈内静脉途径,并结合QRS波群在V1及Ⅱ导联主波方向及起搏阈值对临时起搏电极进行定位,进行了起搏。结果:临时起搏电极一次送入到位,成功率100%,无穿刺失败、血气胸、栓塞、心包填塞、心律失常等并发症。平均手术时间11.1±3.4分钟;平均起搏阈值0.9±0.2V;临时起搏器保留时间4.0±2.24天。结论:经右颈内静脉,在体表心电图指导下,使用普通电极床旁安置心脏临时起搏器,是安全、快捷、有效的方法。Objectives：To explore the technique of implanting tempoary cardiac pacemaker quickly and safely using ordinary electrode via right internal jugular vein at bedside. Methods：12 patients received the therapy of temporary cardiac pacing via right internal jugular vein under directive of superficial ECG and the pacing catheter were located by the pacing threshold and the main wave orientatin in QRS of lead Ⅴ1 and Ⅱ Results：All the pacing electrode were successfully installed on its location by one time, without complications such as puncture failing,hemopneumothorax,embolism,cardiac tamponade and cardiac arrhythmias,etc. The average operation time was 11.1 ± 3.4 minutes, the average pacing threshold was 0.9± 0. 2 volts and the everage pacemaker reserving time was 4.0±2.24 days. Conclusions：The method of bedside temporary cardiac pacing by ordinary electrode via right internal jugular vein under directive of superficial ECG is safe,quick and effective.
