新山地明、新赛斯平、田可在肾移植中疗效和安全性比较

摘　　要：目的比较新山地明胶囊、新赛斯平胶囊、田可胶囊在尸肾移植应用中的疗效和安全性。方法对我院2001年1月-2003年7月接受首次尸肾移植的患者179例,移植术后采用以环孢素A(cyclosporineA,CsA)为主的三联免疫抑制剂治疗,即:CsA+霉酚酸酯(mycophenolatemofetil,MMF)或硫唑嘌呤(azathioprine,Aza)+泼尼松(Prednisone,Pred)。其中新山地明组45例、新赛斯平组53例、田可组81例,随访12-30月,观察3组血药浓度变化、急性排斥反应发生率、人/肾存活率、感染发生率;肾功能、肝功能、血脂及血糖水平在不同时间段变化。用SPSS11.0软件进行统计学处理及初步分析。结果新山地明、新赛斯平、田可CsA血药浓度变化趋势基本相同,急性排斥反应发生率、人/肾存活率、感染发生率的差异无显著性;3组在术后1、3、6、12月肾功能、肝功能、血脂及血糖水平数值变化差异无统计学意义。结论新山地明、新赛斯平、田可3组间的疗效和安全性统计学上无显著差异。Objective To explore the long-term clinical effect and safety of Cyclosporine A （Neoral, New Cyspin and TianKe） in patients receiving cadaveric renal allograft transplantation. Methods 179 recipients who received cadaveric renal allograft transplantation in Peking University Third Hospital from 2001. 01 to 2003. 07 were collected. After renal transplantation, the immunosuppressive therapy including Cyclosporine A ＋ Mycophenolate Mofetil or Azathioprine ＋ Prednisone was administered. The patients were divided into Neoral group （45p）, New Cyspin group（53p） and TianKe group （81p）, and were followed up for 12- 30 months to collect the data of the variation of concentration in whole blood, the incidence of acute rejection, survival rate of patient/graft, incidence of infection, impariment of renal and liver function,lipid and glucose metabolism level in different segment. The data were analyzed by SPSS 11.0 statistical soft ware. Results The variation of blood concentration of Neoral, New Cyspin and TianKe group was similar. There was no significant difference in the incidence of acute rejection, survival rate of patient/graft, incidence of infection among the three groups. The variation of renal and liver function, lipid and glucose metabolism level one month, three month, six months and one year after transplantation had no statistical significance. Conclusion The clinical effect and safety of Neoral, New Cyspin and TianKe group has no statistical significance.
