钩端螺旋体外膜菌苗不同剂量的接种反应与免疫效果观察

摘　　要：本文报告了含有赖型和七日热型抗原的钩端螺旋体（简称钩体）外膜菌苗免疫４００余人，其中１００μｇ组免疫２００余人，２００μｇ组免疫２００余人，同时设菌体菌苗和安慰剂组各２００余人作为对照。结果反应轻微，仅个别对象出现轻微的体温升高和局部红肿反应，无强反应发生。一针免疫后抗体显著升高，阳转率黄疸出血群赖型达９８．１３％以上，七日热群七日热型达９６．２６％以上，ＧＭＴ分别为２２７．０９和５０．８３以上，较菌体菌苗二针免疫抗体高２倍以上，且免疫后３个月下降亦较菌体菌苗为缓。１００μｇ免疫后反应更为轻微，抗体水平满意，建议以每型１００μｇ外膜苗扩大使用。Outer membrane vaccine of leptospira (OMV) was evaluated for safty and immunoginicity in human. All subjects were divided into four groups, each group had about 200 adults and was immunized with 100 μg/dose, 200μg/dose of OMV, two dose whole cell leptospiral vaccine and placebo respectively. The results indicated that a few of fever and local radness were observed, no serious reactions occured. After immunization with one dose OMV, the level of antibodies increased significantly. The Sero-conversion rats for L. icterohaemorrhagiae and L. hebdomadis were 98.1% and 96.26% respectively. GMTs for L. icterohaemorrhagiae and L. hebdomadis were 1∶227.09 and 1∶50. 83, which were two times higher than those immunized with two dose whole cell vaccine. Three months after vaccination, GMTs for the OMV group reduced less than those immunized with two dose whole cell vaccine. Considering that one 100μg dose of OMV could induce satisfactory level of antibodies and adverse reaction of that group were mildest, it was suggested that one 100μg dose of OMV should be used widely.
