抗焦虑治疗减轻术后疼痛的临床研究

摘　　要：目的观察术前抗焦虑治疗对术后疼痛的影响。方法符合纳入标准的78例患者根据一般资料配对后随机分为观察组和对照组,每组39例。术前选用焦虑自评量表(Self Rating Anxiety Scale,SAS)进行评估,疼痛程度用视觉模拟评分(visual analogous scale,VAS)测定,观察组患者均在术前晚及术后6h口服艾司唑仑2-4mg以抗焦虑;对照组则按术前常规处理。2组患者焦虑及疼痛评定于术后48h进行,分别记录由患者根据VAS自评术后最剧烈的伤口疼痛程度;患者术后疼痛次数,术后48h内肌注哌替啶总量。结果观察组术后的焦虑程度为:28．45±4．35,对照组焦虑程度为:37．22±8．63;观察组疼痛程度为:5．86±1．26,对照组疼痛程度为:7．32±1．11;观察组应用哌替定的剂量为:120±30mg,对照组组应用哌替定的剂量为:150±30mg;治疗组疼痛次数:3．25±0．33,对照组疼痛次数为:5．16±0．76;两组间差异有统计学意义(P< 0．05)。结论应用舒乐安定术前抗焦虑治疗有效,可以减轻术后疼痛程度及次数,减少术后哌替定用量。Objective To observe the effect of anti -anxiety treatment before operation after operation. Methods Seventy-eight patients who meet the criterion were matched by individual data, then they were randomly divided into experimental group and control group. Each group of thirty - nine patients. Before the operation each patient was evaluated with self rating anxiety scale. After the operation each patient was evaluated with visual analogous scale. The patients of experimental group ate estazolam（2 -4mg） on the night before the operation and at the sixth hour after the operation. The patients of the control group had a routine operation. Anxiety and pain of all patients were evaluated at the forty-eighth hour after the operation. Each patient evaluated the acute pain by himself after operation with visual analogous scale. The times of pain after operation and the gross of meperidine by muscle among 48 hours after operation were studied. Result The score of self rating anxiety scale in experimantal group was 28.45 ± 4.35 ; the score of self rating anxiety scale in control group was 37.22 ± 8.63. The score of visual arlalogous scale in expermantal group was 5.86 ± 1.26, the score of visual analogous scale in the con- trol group was 7.32 ±1.11. The gross of meperidine in the expermantal group was 120 30mg, the gross of meperidine in the control group was 150 30mg. The time of pain in observe team was 3.25 ±0, 33, the time of pain in control group was 5.16 ± 0. 76. There was significant difference between expermantal group and control group （P 〈 0.05）. Conclusion Anti -anxiety treatment with estazolam is effective before operation. It can inprove the degree and time of pain and reduce the gross of meperidine after operation.
