窒息新生儿血糖动态检测及其护理

摘　　要：目的探讨窒息新生儿血糖的动态变化规律和相应的护理对策。方法动态检测42例窒息新生儿生后1、6、12、24、36、48、72h血糖情况,同时检测30例健康新生儿作为对照。结果重度窒息新生儿血糖生后6h、12h明显升高,其中以生后6h为最高,与对照组和轻度窒息组相比均有显著性差异(P＜0．01)；随后重度窒息新生儿血糖明显下降,到36h降至最低,与对照组和轻度窒息组相比均有显著性差异(P＜0．05)。轻度窒息组与对照组各时间段血糖值比较,差异无统计学意义(P＞0．05)。结论针对窒息新生儿不同时间段血糖变化的特点,应加强对重度窒息新生儿生后72h内血糖的监测,必要时测血糖1次/2h；加强对重度窒息新生儿高、低血糖症状的病情观察；生后14h以内重度窒息新生儿慎用葡萄糖液体补液,忌输高渗葡萄糖液体．12h后可逐步试喂奶,增加喂奶量。Objective To explore the rule of dynamic changes of blood sugar in neonates as well as corresponding nursing strategies. Methods The blood sugar of 42 suffocated neonates （suffocation group） at 1, 6, 12, 24, 36, 48 and 72 hours after birth was monitored dynamically and another 30 healthy neonates were selected as controls. Results The blood sugar of severely-suffocated neonates at 6 and 12 hours after birth and escalated to its pinnacle at 6 hours, showing significant difference compared with the controls and the mildly-suffocated neonates （P〈0.01）. After that, the blood sugar of severely-suffocated neonates began to descend to its minimum at 36 hours, showing significant differences compared with the control group and the mildly-suffocated neonates（P〈0.05）. But no significant differences were statistically found between the controls and the mildly-suffocated neonates at each time point （P〉 0.05）. Conclusion The monitoring of blood sugar of neonates should be enhanced within 72 hours after birth, pertinent to the features of their sugar changes at different time points. If necessary, it should be tested once every two hours. The observation over the symptoms of hyperglycaemia and hypoglycaemia should be enhanced. In 14 hours after birth, it is prudent to use GLU fluid at fluid replacement and hypertonic glucose fluid is forbidden. After 12 hours, the neonates can be fed with mild to add the amount of milk.
