摘 要:目的测定颅脑损伤患者伤后不同时间血清中表皮生长因子(epidermal growth factor,EGF)和C-反应蛋白(C-reactive protein,CRP)的浓度变化,探讨其在继发性颅脑损伤发病机制中的作用及随损伤时间的变化规律,为临床治疗颅脑损伤的新途径提供理论依据。方法测定88例颅脑损伤患者伤后即刻、1天、3天、7天、14天血清中EGF、CRP的含量。结果存活组:颅脑损伤后EGF浓度显著低于正常对照组,且伤情越重,EGF浓度越低。CRP的变化与之正好相反,且两者之间呈现明显的负相关(r=-0.774,P〈0.01);死亡组:在一般死亡病例(生存时限2~14天)中,两者不升高或轻度升高,反应迟滞;在特殊死亡病例(生存时限小于2天)中,两者快速升高而后急速下降直至死亡。结论EGF、CRP浓度与颅脑损伤密切相关,可能参与颅脑损伤后神经元保护及损伤后修复过程;另外,死亡组亦有一定的规律可循;暗示肝脏功能的异常是颅脑损伤后死亡的一个原因。Objective To study the changes of serum levels of EGF, CRP and investigate the pathologic mechanism of secondary craniocerebral injury. Methods 88 cases with acute craniocerebral injury were enrolled, whose serum levels of EGF, CRP were determined at different time. Results Survival group:the serum EGF level in the patients of craniocerebral injury were decreased significantly, and compared with normal controls, which were negative with the order severity. On the contrary, the serum CRP level increased and there was a significantly negative correlation with EGF( r = -0. 774, P 〈 0.01); Death events: in the general cases (survived in 2- 14 d), the serum levels of EGF and CRP had no change or mildly increased and were reacted stagnantly; In special cases (survival time less than 2 d), both of them increased quickly and then declined rapidly until death. Conclusion The results indicated a close relationship between serum levels of EGF, CRP and the degree of craniocerebral injury. They might play a role in neuronal - protection and reparation after craniocerebral injury. In addition, there was a certain regularity in death cases, which suggested the liver's dysfunction might be the dead reason for brain injury cases.
