风疹病毒母婴传播定量PCR阈值与出生婴儿临床表现的相关性研究

摘　　要：目的探讨风疹病毒母婴传播定量PCR阈值与出生婴儿临床表现的相关性研究。方法应用荧光定量PCR技术,对23例ELISA初筛为RV-IgM抗体阳性的孕母及其婴儿血清中RV-RNA含量的检测。结果当孕妇血清中RV-RNA含量小于或等于7.93×102±2.03×102拷贝/μL时,婴儿出生时无临床表现,血清中未检测到RV-RNA;当孕妇血清中病毒含量在3.89×103±1.10×103拷贝/μL时,婴儿出生时可出现不同程度的较为典型的临床表现及迟发表现,血清中即可检测到RV-RNA,组间差异有显著性(P<0.01)。结论孕妇与婴儿血清中RV-RNA含量成正相关,亦与婴儿临床表现成正相关。PCR定量孕妇血清中病毒含量在3.89×103±1.10×103拷贝/μL时,可为导致婴儿感染的参考阈值,这有助于早期诊断和判断预后,这对指导临床具有重要的理论意义和实用价值。Objective： To Relative research between quantitative PCR referenced threshold of Rubella Virus pregnant transmit with abnormal clinical characters of infants. Methods： Fluarogenic quantitative polymerase chain reaction （PCR） was used to detect the levels of RV - RNA in the serum samples of 23 couples of pregnant women and their infants. Results ： When the RV - RNA loads in the serum of the pregnant were less than or equal 7.93×10^2±2. 03×10^2 copies/μL, RV - RNA could not be detected in the serum of the infants, never found any abnormal character when baby - birth, also never detected RV in the serum; When the RV-RNA loads in the serum of the pregnant were 3.89×10^3±1.10×10^3copies/μL, could find the clinical character and obviously when baby - birth; The RVRNA could be detected in the serum. Team samples were found difference significantly （ P 〈 0.01 ）. Conclusion： There are the positive correlated with the percent of the RV - RNA loads in the serum between the pregnant and infants, also the positive correlated with the clinical character in infants. The RV-RNA loads in the serum of the pregnant were 3.89× 10^3±1.10×10^3 copies/μL probably used as the referenced threshold to judge whether it could cause transit RV to the infant during pregnancy. This provides benefit for the early diagnosis and decision prognosis. There are impotsnt theory significance, and practice values to guide clinicas .
