甲状腺功能亢进介入栓塞血管的数量与疗效分析

摘　　要：目的分析介入治疗甲状腺功能亢进(甲亢)栓塞血管的数量与疗效。方法15例甲亢患者均进行两侧甲状腺上下动脉造影,其中1例还进行最下动脉造影,并对相应血管实行栓塞。结果甲状腺动脉增粗甲状腺染色增大者,两上29支(1支甲状腺上动脉手术结扎)、左下1支、右下2支、最下1支。甲状腺淡薄较大染色者,左下6支、右下3支。两甲状腺上动脉栓塞5例、两上+左下5例、两上+右下2例、左上+右下1例、左上+两下+部分右上及最下1例、两下1例。停药9例、少量抗甲状腺药物维持6例。结论栓塞甲状腺动脉的支数应根据甲状腺动脉造影的血供,甲状腺染色大小而定,选择供血丰富的有明显甲状腺染色或淡薄染色的靶血管进行栓塞。Objective To analyze the curative effects on number of embolized arteries in interventional therapy for hyperthyroidism. Methods Superior and inferior thyroid arteriographies were performed for 15 patients with hyperthyroidism including one of the lowest artery and followed by the correlative arterial embolization. Results The interventional proceduce was carried out according to the angiographic demonstration asal stating in the followings：①thyroid arteries enlarged with dense stain： left and right superior 29 embolization（one superior thyroid artery was ligated）, left inferior one, right inferior two, lowest one. ② Thyroid in light stain： left inferior six, right inferior three. The interventional embolization was done as below： five patients： right and left superior thyroid arteries embolized; ③five patients： right and left superior ＋ left inferior thyroid arteries embolized; ④two patients： Right and left superior ＋ right inferior; ⑤one patient： left superior ＋ right inferior; ⑥one patient： left superior ＋ right and left inferior ＋ partial right superior ＋lowest; ⑦ne patient： left ＋ right inferior. Drug administration was discontinued in 9 patients and just a small dose for 6 patients. Conclusions The number of thyroid arteries to be embolized depends on the lesions blood supply providing by thyroid arteriographies better to choose those with dense or light stained ones to be embolized. （J Intervent Radiol, 2006, 15 ： 463-465）
