摘 要:目的探讨外周血白细胞分素-8(IL-8)在窒息新生儿中的变化及临床意义。方法采用ELISA法检测42例不同程眨窒息新生儿不同病期血清的IL-6、IL-8水平,并与正常足月新生儿脐血42例进行对照。结果窒息新生儿第1天血清IL-6为57.6ng/L(中位数,以下同),IL-8为353.3ng/L,明显高于正常脐血对照组的2.0、92ng/L(Z=4.564~5.856,P〈0.01),重厦窒息组患儿血清IL-6、IL-8分别是87、579.6ng/L,分别高于轻度组(52.6、336.3ng/L)(Z=2.228-2.702,P〈0.05)。血清IL-6、IL-8水平间呈显著正相关(d=0.59~0.72,P〈0.01)。窒息组患儿动态观察发现,病程第3天时血清IL-6(56.6ng/L)、IL-8(320.6ng/L)水平较第1天时(分别为57.6、353.3ng/L)差异均无显著性(Z=0.314~1.372,P〈0.05):第7天时血清IL-6(14.6ng/L)、IL-8(238.4ng/L)水平均明显低于第1天和第3天时(Z=2.455,P〈0.05,Z=3.622,P〈0.01)。结论蕈息新生儿血清中IL-6、IL-8水平明显增高,在新生儿窒息发病机制中起重要作用。Objective To explore the change of IL-6 and IL-8 in asphyxia newborn and clinical significance. Methods Compared with the control group,ELISA method was used to determine the degree of IL-6 and IL-8 in different asphyxia newborn and in different ,'oures of disease, Results In asphyxia newborn group serum IL-6 ( 57.6 ng/L) and IL-8 ( 353.3 ng/L) were higher than the control group ( 2. 0,92 ng/L) ( Z = 4. 564 - 5. 856,P 〈 0. 01 ) in the first day, heavy asphyxia group serum IL-6 ( 87 ng/L) and IL-8 ( 579. 6 ng/ I,) were higher than light asphyxia group ( 52. 6,336. 3 ng/L) ( Z = 2. 228 - 2. 702, P 〈 0. 05 ) and had obviously positive correlation ( u =0. 59 -0. 72,P 〈0, 01 ). Serum IL-6(56.6 ng/L) and IL-8(320. 6 ng/L) did not have obviously difference compared with the first qlay on the third day( Z =0. 314 -1. 372,P 〈0, 05) ;sernm IL-6( 14. 6 ng/L) and IL-8( 238.4 ng/L) were lower than aboues on th+, seventh day (Z=2.455,P〈0.05,Z=3.622,P〈0, 01 ). Conclusion Serum IL-6 and IL-8 degree improved apparently in asphyxiu newborn which had important value in newborn asphyxia pathogenesis.
