心肌速度分布图对肥厚型心肌病左心室局部收缩功能的研究

摘  要:目的应用组织多普勒心肌速度分布图(myocardial velocity profile,MVP)评价肥厚型心肌病(hypertrophic cardio myopathy,HCM)左室短轴方向局部收缩功能。方法记录23例室间隔非对称性HCM患者和15例健康志愿者室间隔和左室后壁短轴方向的心肌运动速度,得出MVP和心肌速度梯度(myocardial velocity gradient,MVG)。结果HCM组室间隔和左室后壁收缩期MVG均显著低于对照组,分别为[(3.6±0.4)s-1对(1.2±0.4)s-1,P<0.0001;及(5.1±1.2)s-1对(2.4±0.6)s-1,P<0.001],以室间隔更为显著。HCM组和对照组室间隔和左室后壁的MVP曲线均呈近似线性,但前者心肌运动速度分布较后者明显分散。结论HCM患者左室短轴方向局部收缩功能较正常人减低,MVP能准确地反映短轴方向心肌在心动周期中的运动速度,不仅可得出MVG,还可显示心肌各点速度分布情况,为临床提供了一种准确评价左室短轴方向局部收缩功能的新方法。Objective To evaluate the regional left ventricular systolic function in short-axis orientation by myocardial velocity profile(MVP) in patients with hypertrophic cardiomyopathy(HCM). Methods Transmural wall-motion velocities in the ventricular septum and left ventricular posterior wall were recorded in 23 patients with asymmetric septal hypertrophy and compared with 15 healthy volunteers. Their MVPs and myocardial velocity gradients (MVG) were determined to evaluate the regional left ventricular systolic function in short-axis orientation. Results The systolic MVGs in the ventricular septum and left ventricular posterior wall were significantly lower in HCM group than in control group [(3.6 ± 0.4) s^-1 vs (1.2±0.4)s^-1, p〈0.0001 and (5.1± 1.2)s^-1 vs (2.4±0.6)s^-1, P〈0.001, respectively], which were more significantly lower in HCM group. In both groups, the MVPs in the ventricular septum and left ventricular posterior wall were closely linear, but the distribution of velocities in the ventricular septum was fairly dispersed in HCM group compared with control group. Conclusions The regional left ventricular systolic function in short-axis orientation was decreased in patients with HCM. The MVPs and MVGs obtained from two dimensional tissue Doppler imaging may represent new indices for evaluating regional left ventricular contractile abnormalities in patients with HCM.
