摘 要:目的研究丹参酮对原发性高血压(EH)病人病程早期外周血单核细胞(PBMC)激活的作用。方法选择病程<5年且无心、脑、肾、血管等靶器官损害的EH病人30例,并设正常对照,通过体外细胞培养,血管紧张素Ⅱ(AngⅡ)刺激以及丹参酮干预,采用显微镜-计算机下细胞计数方法及双抗体夹心酶联免疫吸附实验(ELISA)和逆转录-聚合酶链反应(RT-PCR)等技术,检测PBMC的黏附活性及其分泌和表达炎性细胞因子的水平。结果EH病人PBMC与内皮细胞的黏附数及其分泌和表达肿瘤坏死因子α(TNF-α)、白细胞介素-1β(IL-1β)、白细胞介素-6(IL-6)等炎性细胞因子的浓度和mRNA水平在基础状态下同对照组相当,经AngⅡ刺激后却显著高于对照组,丹参酮干预可使2组PBMC的黏附及分泌活性降至同一水平。结论EH病人病程早期PBMC处于预激活状态,丹参酮能抑制预激活的PBMC进一步激活。OBJECTIVE To investigate the effects of tanshinone on peripheral blood monocytes(PBMC)activation in patients with essential hypertension( EH)during early stage of the diseased course. METHODS 30 patients with EH less than 5 years and undetectable impairment of target organs such as cardiac,brain, kidney and blood vessel, and 30 normotensive healthy individuals used as a control group were involved in the study. PBMC were isolated and cultured in vitro, stimulated with angiotensin Ⅱ ( Ang Ⅱ ) and pretreated with tanshinone. PBMC adhesion to endothelial cells, proinflammatory cytokines secretion and expression were determined by cell-counting assay with computered microscope, as well as sandwich enzyme-linked irmnunosorbent assay(ELISA) and reverse transcription-polymerase chain reaction( RT-PCR). RESULTS PBMC adhesion to endothelial cells, and the secretion and expression of proinflammatory cytokines such as tumor necrosis factor-α (TNF-α), interleukin-1β( IL-1β)and interleukin-6(1L-6) were similar in two groups under basic condition, and increased more significantly in hypertension group than in control group after the stimulation with Ang Ⅱ. Pretreatment with tanshinone before exposure to Ang Ⅱ diminished PBMC adhesion and secretion activities in patients with EH and healthy individuals to a normal levels. CONCLUSION PBMC are preactivated in patients with EH during early stage of the diseased course. Tanshinone can inhibit further activation of preactivated PBMC.
