摘  要:目的:探讨三七皂甙在烧伤休克延迟复苏大鼠中的抗渗作用及对肿瘤坏死因子α、白细胞介素1β、白细胞介素6的调控。方法:实验于2003-12/2005-02在第二军医大学长海医院烧伤中心实验室完成。选用36只健康成年的SD大鼠,采用随机数字法分为假烫组、烫伤组及治疗组,每组12只。①制备烧伤模型:用恒温水烫仪以100℃水烫背部13s,造成大鼠30%TBSAⅢ度烧伤(病理证实)。②制备烧伤休克延迟复苏模型:烫伤大鼠均于伤后6h腹腔注射乳酸林格液进行延迟复苏,总量按Parkland公式4mL/(kg·%)计算。③治疗组伤后0.5h腹腔注射三七皂甙(100mg/kg);假烫组大鼠以38℃水模拟烫伤过程。④伤后24h采用干湿重法检测肺脏、肝脏及肠的含水量,组织含水量(%)=(湿重-干重)/湿重×100%。测定各上清液相应的伊文思蓝浓度,以此来表示各脏器血管通透性的大小,伊文思蓝含量(μg/g干重组织)=伊文思蓝浓度(mg/L)×10/[1-组织含水量(%)]。采用酶联免疫吸附法(ELISA方法)分别测定血浆中肿瘤坏死因子α、白细胞介素1β及白细胞介素6的水平。结果:36只大鼠均进入结果分析。①伤后24h治疗组大鼠的肺脏、肝脏及小肠的含水量明显低于烫伤组相应脏器含水量((79.91±1.76)%比(82.04±2.05)%,(71.72±1.75)%比(73.63±1.61)%,(76.52±1.43)%比(80.31±3.69)%,P<0.05)。②治疗组大鼠的肺脏、肝脏及小肠的血管通透性(伊文思蓝μg/g干重组织)明显低于烫伤组大鼠相应脏器血管通透性(313.05±19.30比457.86±43.33,156.72±17.75比243.63±26.74,236.52±26.43比380.31±33.69,P<0.01)。③治疗组大鼠血浆中肿瘤坏死因子α、白细胞介素1β及白细胞介素6的水平均显著低于烫伤组血浆中各细胞因子水平[(258.91±20.4)ng/L比(612.30±75.03)ng/LQ(101.37±29.86)ng/L比(208.45±49.04)ng/LQ(433.65±31.78)ng/L比(726.46±38.61)ng/L,P<0.01]。结论:大鼠严重烧伤后,早期使用三七皂甙,能下调肿瘤坏死因子α、AIM: To investigate the anti-permeability effect of sanchinoside in rats with delayed resuscitation of bum shock as well as its regulation effect on tumor necrosis factor-α (TNF-α), interleukin-1β(IL-1β) and interleukin-6 (IL-6). METHODS: The experiment was conducted in the Central Laboratory of Bums of Changhal Hospital in Shanghai between December 2003 and February 2005. Thirty-six healthy adult SD rats were adopted and randomly divided into sham-bum group, bum group and treatment group with 12 rats in each group. ①Establishment of bum models: The dorsal skin of the rats was exposed to 100 ℃ water with template device for 13 s, so as to get full-thickness dermal bums comprising 30% TBSA (pathologically proved). ②Establishment of models of delayed resuscitation of bum shock: rats were delayed to resuscitate by peritoneal injection with Ringer lactated solution at 6 hours after bum according to Parkland formula [4 mL/(kg·%)].③At 30 minutes after bum, rats in the treatment group were intrapertoneally injected with sanchinoside at 100 mg/kg, and rats in the sham-bum group were immersed into 38 ℃ water to simulate the bum process. ④The water content of lung, liver and intestine were detected by dry-wet weight method. The water content of tissue (%)=(wet weight-dry weight)/wet weight ×100%. The concentration of corresponding Evans blue in above-mentioned supematant were determined to manifest the vasopermeability of all organs. The content of Evans blue μg/g dry weight of organ)= the concentration of Evans blue (mg/L)×10/[1-the water content of organ (%)]. The levels of TNF-α, IL-1βand IL-6 in serum were assayed with ELISA method respectively. RESULTS: A total of 36 rats were involved in the analysis of results,① The water contents of lung, liver and intestine in rats of the treatment group were obviously lower than those in the bum group at 24 hours after bum ((79,91±1,76)% vs.(82,04±2,05)%,(71.72±1.75)% vs, (73.63±1.61�
