病毒性脑炎患儿血清和脑脊液可溶性L-选择素表达的意义

摘　　要：目的探讨可溶性L-选择素(sL-selectin)在病毒性脑炎(VE)发病机制中的作用及其意义。方法用酶联免疫吸附双抗体夹心法测定30例VE患儿(轻症组14例,重症组16例)急性期及恢复期及10例对照组儿童血清和脑脊液sL-selectin的质量浓度。结果VE患儿急性期血清和脑脊液sL-selectin质量浓度均较对照组明显升高(P均<0.01),且重症组明显高于轻症组(P<0.01)。恢复期轻症组与重症组均明显低于急性期(P均<0.01),重症组血清和脑脊液sL-selectin质量浓度仍较对照组明显升高(P<0.05,P<0.01),而轻症组与对照组比较无统计学差异(P>0.05)。结论L-selectin参与VE的发病过程。血清和脑脊液sL-selectin质量浓度对于判断VE病情轻重、评估预后及指导治疗有重要参考价值。Objective To observe the changes of soluble L- seleetin（sL- selectin） concentrations in children with viral encephalitis （VE） and probe its significance in pathogenesy and clinic. Methods Selecting 30 children with VE, including 14 mild case and 16 severe case. Collecting their venous blood and cerebrospinal fluid（CSF） in acute and recovery stage. Using double antibody sandwich enzymelinked immunosorbent assay（ELISA） method to measure SL- selectin concentrations of serum and CSF and comparing with control group, respectively. Results SL- selectin concentrations from serum and CSF in the acute stage were significantly higher than those of control group（all P 〈0.01） and its concentrations of the severe group were significantly higher than those of the mild group（ P 〈0.01 ）. In recovery phase, sL - selectin concentrations from serum and CSF were significantly lower than those of in the acute stage（all P〈 0.01 ） and sL-selectin concentrations of the severe case were still significantly higher than those of control group （P〈 0.05, P〈 0.01 ）;but in the mild case,its concentrations were not different compared with control group significantly（P 〉 0.05）. Conclusions L- selectin may participate in the pathologic course in VE. The concentrations of sL- selectin from serum and CSF are related with the patients＇ condition and it has important reference value in judging patients＇ condition,assessing their prognose and guiding clinical treatment.
