湖北省2001～2005年老年肺结核疫情分析

摘　　要：目的了解湖北省2001～2005年老年肺结核患者疫情状况和耐药情况。方法收集2000年湖北省第四次结核病流行病学抽样调查资料和2001～2005年新涂阳肺结核病人登记以及湖北省结核分支杆菌耐药监测资料,分析老年肺结核的流行状况,并与其他年龄组进行比较。结果≥65岁年龄组的老年活动性肺结核患病率,涂阳患病率和菌阳患病率分别为1966.7/10万,378.2/10万和756.4/10万,为各类病人平均患病率的4.3倍、2.4倍和3.7倍;2001～2005年,共登记新涂阳肺结核111307例,其中≥65岁组13332例(12.0%);≥65岁年龄组初始耐药率和初始耐多药率高峰均高于不同年龄组平均水平(χ2=14.1,P<0.01;χ2=4.9,P<0.05);获得性耐药和获得性耐多药率各年龄组间差异无显著性(χ2=0.42,P>0.05;χ2=0.76,P>0.05)。结论湖北省老年肺结核患病率高,病例构成比大,而新涂阳病例登记比例偏低,耐药状况严重,建议加强老年肺结核发现和治疗管理。Objective To explore the epidemical status of pulmonary tuberculosis in the elderly in Hubei Province. Methods The data of the fourth tuberculosis survey in Hubei in 2000, the registered new smear positive tuberculosis patients and results of surveillance of tuberculosis from 2001 to 2005 were analyzed. Results The incidence of active, smear - positive and bacteria - positive tuberculosis in the age group of ≥65 years were 1966.7/100 000, 378.2/100 000 and 756.4/100 000, about 4.3, 2.4 and 3.7 times the average incidence of other diseases. From 2001 - 2005, 111 307 new smear- positives were registered, among which 13 332 （12.0%） were over the age of 65 years old; The total drug resistance rate and total multi - drug resistance （MDR） were 39. 2% and 8.2% . The initial drug resistance rate and initial MDR rate in ≥65 were significantly higher than other age groups （x^2 = 14.1, P 〈 0.01;x^2 = 4.9, P 〈 0.05）. there no significant difference in acquired drug resistance rate and acquired MDR rate were observed among other age groups （X^2 = 0.42, P 〉 0.05 ; X^2 = 0.76, P 〉 0.05 ）. Conclusion The incidence rate of pulmonary tuberculosis in Hubei Province in the elderly is high and the status of drug resistance is serious. Thus the management of identification and treatment of pulmonary tuberculosis be strengthened.
