摘 要:目的通过检测不同时期糖尿病肾病患者辛伐他汀治疗前后血清转化生长因子(TGF)-β1水平的变化,探讨其与糖尿病肾脏病变之间的关系,以及他汀类药物治疗糖尿病肾病的机制。方法根据尿白蛋白排泄率将47例糖尿病患者分为糖尿病无肾病组(A组,22例)和糖尿病肾病组(B组,25例),糖尿病肾病组接受辛伐他汀治疗。23例健康者为对照组。结果糖尿病无肾病组和糖尿病肾病组患者血中TGF-β1水平分别为147 ng/m l±23 ng/m l和172 ng/m l±18 ng/m l,明显高于对照组的132 ng/m l±36 ng/m l,其中肾病组明显高于无肾病组。使用辛伐他汀治疗后,糖尿病肾病组患者血中TGF-β1水平逐渐降低,尿白蛋白排泄率下降。结论TGF-β1在糖尿病肾病的发病过程中起着重要作用,辛伐他汀治疗糖尿病肾病的机制可能是通过抑制TGF-β1的表达实现的。Objective To investigate the significance of changes in the levels of transforming growth factor β1 ( TGF-β1 ) after treatment with simvastatin in patients with diabetic nephropathy. Methods 47 patients with diabetic mellitus were divided into two groups according to urinary albumin excretion rate, of whom 22 were diabetic patients without nephropathy ( group A) and 25 patients with diabetic nephropathy (group B). The patients in group B were treated with simvastatin. The levels of TGF-β1 were detected with enzyme linked immunosorbent assay (ELISA) ,and the levels of urinary albumin were detected with radioimmunoassay(RIA). 23 healthy subjects were used as controls. Results The levels of the serum TGF-β1 in the diabetic patients were significantly higher than those in controls. After the treatment with simvastatin,the levels of the serum TGF-β1 and the urinary albumin excretion rate in group B decreased gradually. Conclusion TGF-β1 may play an important role in development of diabetic nephropathy. Simvastatin may achieve its therapeutic effects in diabetic nephropathy by inhibiting the expression of TGF-β1.
