经尿道选择性绿激光前列腺汽化术对BPH患者性功能的影响

摘　　要：目的:探讨经尿道选择性绿激光前列腺汽化术(PVP)治疗BPH对患者术后性功能的影响。方法:采用PVP治疗BPH患者56例,应用国际勃起功能障碍问卷-5(IIEF-5)及自制调查表包括性欲、阴茎勃起硬度、逆行性射精情况等,对手术前、后的性功能状况进行对比。结果:随访获得完整填表52例,随访率92.8%(52/56)。术前性功能正常的患者49例,术后患者均保留性欲和满意的性生活。治疗前IIEF-5评分22.57±1.64,治疗后评分21.82±2.58,手术前后比较差异无统计学意义。术后精液量明显减少者占36.7%(18/49),发生逆行性射精者占24.5%(12/49),阴茎勃起硬度下降者占10.2%(5/49),与术前比较差异均有统计学意义(P<0.05)。结论:PVP与其它前列腺手术相比,并发症少,对性功能的影响较小;但术后精液量明显减少、逆行性射精和阴茎勃起硬度下降,临床应予以重视。Objective：To investigate the effect of greenlight photoselective vaporization of prostate （PVP） on sexual function of elderly male patients with benign prostatic hyperplasia （BPH）. Methods.. International erectile dysfunctions questionnaire （IIEF-5） and a self-designed ejaeulatory questionnaire were used in this study. The sexual function changes of were assessed in followed-up 56 patients with BPH before and after PVP.. sexual interest, erectile consistency, retrograde ejaculation and erectile dysfunction （ED）, and the results of the sexual function before and after operation were compared. Results：In 49 patients who were potent before operation, no patients had newly developed impotence. The questionnaire result of postoperative IIEF-5 was no significantly than preoperative score （P〉0.05）. The ejaeulatory volume of 18 patients were evidently reduce 36. 7% after PVP, 24. 5% （12/49） patients were found with retrograde ejaculation, the incidence of decreased erectile stiffness was 10.2 % （5/49） respectively compared with pre-operation sexual functions at the 6th month. These data were significantly compared with pre-operation （P〈0.05）. Conclusions：PVP has a little impact on sexual function in men with BPH. The evident change of sexual function after PVP include the ejaculum reduce, retrograde ejeulation and erectile consistency decrease, which must be taken into consideration in clinical practice.
