慢性鼻窦炎、鼻息肉术后复发原因及再手术探讨

摘　　要：目的:探讨鼻内镜手术失败的原因,评价再手术的时机。方法:对67例慢性鼻窦炎、鼻息肉复发患者再次施术并分析其复发原因。术前常规行鼻内镜检查和鼻窦CT扫描,术后随诊半年以上。结果:病变彻底清除,窦口开放良好,痊愈52例(77.6%);3例(4.5%)术中出血较多,手术被迫停止,12例(17.9%)病变清理不彻底,术后3个月复发。结论:慢性鼻窦炎、鼻息肉术后复发原因主要为术前、术后未规范治疗,鼻腔鼻窦解剖结构未很好恢复,病变未彻底清除,病变为鼻息肉病。再次鼻内镜手术效果满意,但要间隔半年以上。Objective： To explore the reasons of failed endoscopic sinus surgery and to evaluate the time for reoperation. Method： Endoscopic sinus surgery was re-performed on 67 patients, and the reason of relapse was analyzed. Results： During a follow-up over 6 months, 52 cases were cured, 3 stopped operating as serious bleeding, and 12 relapsed 3 months after the surgery. Conclusion： The reason of reeurrenee is that pre- and post-operative treatment is not normal, the anatomic structure of nasal cavity and parasinus is not entirely recovered, and the diseases and polyposis are not entirely excised. Reoperation should be performed at least six months after first operation.
