无创通气在COPD合并呼吸衰竭治疗中的应用

摘　　要：目的探讨双水平正压无创通气在COPD合并呼吸衰竭中的治疗作用。方法64例COPD合并呼吸衰竭患者随机分为B iPAP呼吸机无创通气组32例与常规治疗对照组32例,监测其治疗前、治疗后2、12、72 h血气变化,观察临床情况的变化及并发症的发生,对结果进行对比分析。结果无创通气组的pH、PaO2、PaCO2等指标明显改善(P<0.0 1,P<0.05)。结论无创双水平正压通气能明显改善COPD并呼吸衰竭患者的低氧血症及高碳酸血症,其疗效优于常规治疗方法。Objective To explore the effects of non - invasive ventilation on pulmonary gas exchange during acute exacerbations of chronic obstructive pulmonary disease with respiratory failure. Methods 64 COPD patients with respiratory failure were randomly divided into ventilation group （32 cases） and control group （32 cases）. Measurements of pulmonary blood gas analysis were made in （ 1 ） breathing spontaneously （baseline）; and （2） after 2 h, 12 hand 72 hofBIPAPusingafull-facemask. Results There were significant differences in arterial blood gas after 2 h, 12 h and 72 h between the two groups. The situation of the patients in the ventilation group improved soon after the treatment. Conclusion Non-invasive bi-level positive airway pressure ventilation can be successfully used in COPD patients with respiratory failure to improve hypoxemia and hypercapnia in comparison with traditional therapy.
