国产Amplatzer封堵器治疗先天性心脏病28例疗效观察

摘　　要：目的：探讨应用国产Amplatzer封堵器治疗先天性心脏病（CHD）的临床疗效。方法：选择28例CHD患者（男16例，女12例，年龄4～32岁），其中动脉导管未闭10例，房间隔缺损15例，室间隔缺损3例，在经胸彩色超声心动图（TTE）及X线影像指导下，经皮沿导丝经传送鞘管将合适的Amplatzer封堵器至缺损处行封堵。结果：①TTE测量房间隔缺损为（13．8±6．5）mm：所用封堵器为（18．6±8．9）mm，术后即刻TTE未发现分流；②左室造影测量室间隔缺损直径为（6．9±2．9）mm，封堵器（8．9±3．4）mm，无1例存在残余分流，术后即刻完全封堵率100％；③造影测量动脉导管未闭最窄处直径为（7．3±2．8）mm，所选用封堵器为（11．2±4．2）mm，封堵成功率100％，随访2月至69月，仅1例有少量分流。结论：应用国产Amplatzer封堵器经皮介入治疗先天性心脏病操作简单、成功率高、安全可靠、近期临床疗效好。Objective： To evaluate the therapeutic efficiency of Chinese - made Amplatzer occluder device in treating congenital heart diseases （CHD）. Methods： 25 patients （male 16cases, female 12 cases, age 4 -32 years old, PDA 10 cases, ASD 15 cases, VSD 3 cases） were treated with the occluder devices through the percutaneous procedure under fluorescopy, angiography and transthoracic echocardiography （TTE）. Results： ①The atrial septal defect （ASD） diameter was （13.8±6. 5） mm by rITE. The diameter of the occluder selected was （ 18.6 ± 8.9 ） mm. No residual shunts were found in all cases after the procedure. ②The ventricular septal defect （VSD） diameter was （6. 9 ± 2. 9） mm measured by left ventricular angiography and the diameter of occluder selected was （8. 9 ±3.4） ram, The success rate was 100% and no residual shunts were found in all cases. ③The narrowest diameter of the patent ductus arteriosus （PDA） was （7. 3 ± 2. 8 ） mm by angiography. The diameter of occluder selected was （ 11. 2 ± 4.2 ） mm, success rate 100%. Only one patient had small shunts. Conclusion： The operation with occluder devices made by China is simple with a high success rate of placement, a high safety and reliability and good occlusion effect.
