摘  要:目的:分析原发性高血压患者不同动态血压负荷(BPL)对左室结构及左室收缩和舒张功能的影响。方法:将106例原发性高血压患者根据动态血压负荷值分为三组,计算左室重量指数(LVMI)、相对室壁厚度(RWT)和左室功能参数。应用单因素方差分析来比较不同组间各参数的差异。结果:LVMI和RWT在动态血压负荷增高组增大,左室收缩功能(mFs代表)与左室舒张功能(E/A比值代表)随动态血压负荷增高而渐次降低。结论:动态血压负荷异常增高加重了左室肥厚和心功能损害程度,40%和70%的动态血压负荷值可作为高血压左室结构和功能的危险分层指标。Objective: To evaluate the impact of ambulatory blood pressure iload(BPL) on left ventricular geometry, systolic and diastolic functions in essential hypertensive patients. Method: 106 cases of essential hypertensive patients were divided into three groups according to the values of BPL. The left ventricular mass index (LVMI) , relative wall thickness(RWT) and the left ventricular function parameters were measured. One- way ANOVA was performed to analyze the differences between groups. Results: LVMI and RWT were increased in higher BPL groups. The left ventricular systolic function (represented by mFs) and diastolic function (represented by E/A) were decreased accompanying the increased BPL. Conclusion:The unusually increased BPL may ANOVA was performed to analyze the differences between groups. Results: LVMI and RWT were increased in higher BPL groups. Conclusion:The unusually increased BPL may contribute to the accentuated damage of left ventricular hypertrophy and left ventricular functions in essential hypertension. The BPL value of 40% and 70% were provided the risk factor of the left ventricular geometry and functions.
