癌胚抗原和增殖细胞核抗原与中耳胆脂瘤上皮

摘　　要：目的观察癌胚抗原、增殖细胞核抗原在中耳胆脂瘤上皮中的表达情况,分析其在胆脂瘤上皮增殖演变过程中的作用。方法应用免疫组化SP染色方法和计算机图像分析系统,检测癌胚抗原、增殖细胞核抗原在32例胆脂瘤上皮和14例胆脂瘤患者外耳道皮肤中的表达情况,并做比较。结果增殖细胞核抗原在胆脂瘤上皮各层细胞均存在较强表达,主要定位在胞核,染色由基底层向表皮层逐渐减弱,外耳道皮肤表达较弱,主要集中在基底层及基底上层；癌胚抗原主要在胆脂瘤上皮的颗粒细胞层及表皮层表达,少数在外耳道皮肤的基底层表达。各组间胆脂瘤上皮各指标阳性细胞率和光密度值均高于外耳道皮肤,和外耳道皮肤间的比较有高度差异性。皮下结缔组织伴随着大量的淋巴细胞和浆细胞,中性粒细胞和巨噬细胞,与胆脂瘤上皮中的阳性率呈一致趋势。同一患者胆脂瘤上皮中癌胚抗原、增殖细胞核抗原高表达,利用SPSS10．0软件包进行统计学分析,存在正相关关系(P＜0．01)。结论癌胚抗原、增殖细胞核抗原在胆脂瘤上皮的表达从不同方面反映了胆脂瘤上皮的过度增生和角化,二者之间可能有协同作用,共同促进胆脂瘤上皮细胞增殖与分化；上皮下炎症反应可能是角化细胞过度增殖的一个原因,为胆脂瘤发病机制提供新的理论依据,便于指导临床。OBJECTIVE To detect the expression and localization of carcinoembryonic （CEA） and proliferative cell nuclear antigen（PCNA）, in human middle ear cholesteatoma,and to evaluate their roles in the proliferation of cholesteatoma epidermis. METHODS The specimens of the middle ear cholesteatoma tissue from 32 cases and external ear skin from 14 cases were examined by immunohistochemical S-P method. The CEA and PCNA expression was detected with computer image analysis. RESULTS All 32 samples of cholesteatoma showed a stronger expression of PCNA than that in the external ear skin. CEA demonstrated the presence of proliferating cells in the epidermal and bead cell layers. There were significant differences between cholesteatoma tissue and the normal external ear skin. The expression rate of CEA and PCNA also showed a significant difference in the two groups. The expression was accompanied with numerous lymphocytes, plasmocytes,neutrophils and macrophages. The high expression of CEA was correlated with high expression of PCNA （P〈0.01）. CONCLUSION The detection of the expression of CEA and PCNA were correlated with the high proliferation of cholesteatoma.
