摘  要:目的通过测量婴儿髋关节α角、β角和股骨头骨性髋臼覆盖率(MR),探讨超声检查在婴儿发育性髋关节异常早期诊断中的意义.方法采用高频5.0~7.5MHz线阵探头,按Graf方法对临床诊断为发育性髋关节异常婴儿17例19侧髋进行超声检查,并按月龄设立正常对照组17例19侧髋关节.结果超声声像图可以较好地再现髋关节的解剖结构.正常对照组髋关节α角、β角和MR值分别为65.3°±4.5°,40.6°±7.5°和(64.2±5.2)%;发育性异常髋关节组α角、β角和MR值分别为48.4°±6.2°,53.0°±10.1°和(40.6±10.8)%.与正常组比较反应骨性髋臼的α角及股骨头覆盖率(MR)减小,反应软骨髋臼发育情况的β角增大.同时超声还能显示X线不能显示的关节唇盂及髋臼内其他软组织.结论超声检查简便快速、无侵袭性,在婴儿发育性髋关节异常的诊断及治疗随访中具有重要意义.AIM: To investigate the value of ultrasonic examination for earlier diagnosis of developmental dislocation of the hip (DDH) by measuring the uhrasonography of infant hip et angle, β angle and acetabular coverage of femoral head ( MR value ). METHODS: 5.0- 7.5 MHz linear transducer was used for the frontal ultrasonographic sectional study of infant hip according to Graf's method. In 17 cases suspected as developmental dysplasia of hip, 19 hips underwent ultrasonic examination, and normal control group was set up according to month age. RESULTS: Ultrasonic examination basically demonstrated the anatomic structure of frontal profile of infant hip. In normal control group, ct angle was 65. 3° ±4.5°, β angle 40. 6° ±7.5°, MR value (64.2 ±5.2)% ; in DDH group, α angle 48.4° ±6.2°, 13 angle 53.0° ± 10. 1°, MR value (40.6 ± 10. 8)%. Compared with normal group, MR and α angle reflecting the development of osseous acetabulum decreased, and β angle reflecting the development of cartilaginous acetabulum increased. Ultrasound also displayed glenoid labrum and other soft tissues of acetabulum which couldn't be displayed by X-ray. CONCLUSION: Ultrasonagraphy of the infant hip is a convenient, fast, non-invasive method for earlier diagnosis and follow-up examination of infant DDH.
