摘  要:目的探讨阴茎勃起障碍(ED)患者螺旋动脉(HA)形态及血流动力学特征。方法用彩色多普勒超声(CDUS)检测60例ED患者HAs形态及血流动力学情况。结果(ICI)后所有病例HAs均能清晰显示。非血管性ED组均可观察到HAs三级分支,静脉性ED组HAs显示二级或三级分支,而动脉性ED组HAs分支明显减少;动、静脉性ED组HAs的收缩期峰值流速(PSV)、舒张末期流速(EDV)及阻力指数(RI)与非血管性ED组存在显著差异(P<0.05),静脉性ED组RI明显低于动脉性ED组(P<0.05)。结论HAs在阴茎勃起过程中发挥着重要作用,其形态及血流动力学特征对CDUS评估ED有重要意义。Objective To explore the features of morphology and hemodynamics of the helicine arteries (HA) using color Doppler ultrasonogrophy in patients with erectile dysfunction. Methods A total of 60 patients with ED underwent CDUS. The morphology and hemodynamics of the cavernosal artery (CA) anti HAs were measured. Results The CAs and HAs were visualized clearly and could be evaluated by CDUS after vasoactive agents intra-cavernous injection in all patients. In the non-vasculogenic group the HA were presented with 3 grades of distal ramification originating from the cavernous arteries; the HAs revealed 2 or 3 grades of distal ramification arteriols in venogenic ED group, while in the patients with arteriogenic ED, the HAs showed obviously reduced ramification originating from the cavernous arteries with irregular calibers. The PSV (peak systolic velocity), EDV (end diastolic velocity) and RI (resistance index) of helicine arteries in arteriogenic and venogenic ED groups had significant difference with those in the non-vasculariogenic ED group (P〈0.05) and the RI of HA in venogenic ED group was lower than that in arteriogenic ED group (P〈0.05) . Conclusions HAs played an important role in the process of penis erection, and the features of morphology and hemodinamics of the helicine arteries were significant for the assessment of ED.
