摘  要:目的探讨可溶性细胞表面分化抗原40配体(sCD40L)水平、核转录因子PPARγmRNA表达与冠状动脉病变的关系及其在冠心病发病中的意义。方法经冠状动脉造影确诊的冠心病患者,ELISA法检测血浆中sCD40L含量,RT-PCR法测定外周血中PPARγmRNA表达,并对冠状动脉病变程度进行分型,分析sCD40L水平、PPARγmRNA表达与冠状动脉病变程度的相关性。结果CHD组与对照组相比sCD40L明显增加(19.0±6.6vs13.4±3.8),PPARγmRNA明显降低(0.40±0.12vs0.62±0.13),尤以ACS组明显,且随冠状动脉病变累计支数越多、病变越严重,sCD40L水平增高,PPARγmRNA表达减少越显著。相关分析显示PPARγmRNA表达和sCD40L水平呈明显负相关(r=-0.43,P<0.001)。结论sCD40L对ACS不稳定斑块有促进作用,PPARγ具有抑制sCD40L的作用,提示通过激活PPARγmRNA表达和降低sCD40L水平可能对冠心病高危人群具有保护作用。[Objective] To investigate the mechanism of sCD40L, peroxisome proliferator-activated receptorγ (PPAR-γ)mRNA in coronary heart disease(CHD) by the relationship study among sCD40L, PPAR-γ and the severity of coronary lesions in coronary heart disease patients. [Methods] 98 patients with CHD who undergone coronary angiography were admitted, Enzyme Linked Immunosorent Assay was used to measure the level of sCD40L, the expression of PPAR-γ mRNA in lymphocytes located in peripheral blood was detected by using RT-PCR. The severity of coronary artery lesions was analyzed. The lesions of coronary artery was typified. [Results] sCD40L level was significantly higher in CHD patients than those in controls [(19.0±6.6) vs (13.4:±3.8)]. Significant decreasing is observed in CHD patients in PPAR-γ mRNA [(0.40±0.12) vs (0.62±0.13)]. The more the number of coronary artery lesions and severity of coronary artery lesions, the higher the level of sCD40L and the lower the expression of PPAR-γ mRNA. Negtive correlation were seen between PPAR-γmRNA and sCD40L level. [Conclusions] sCD40L accelerated the formation of unstable plaque, PPAR-γ inhibited the activation of sCD40L, PPAR-γ is a negtive regulator of coronary artery lesions. It will be a valuable method to protect patients from the incidents of coronary artery disease by acting the expression of PPAR-γ and decreasing the level of sCD40L.
